体征和症状

诊断
根据患者病史和体格检查，经常怀疑肩关节脱位的诊断。制作射线照片以确认诊断。大多数位错在X线片上显而易见，显示盂肱关节不一致。在标准AP射线照片上可能难以检测到后位错，但在其他视图上更容易检测到。减少后，通常重复X光片以确认成功减少并检测骨损伤。在反复肩关节脱位后，可以使用MRI扫描来评估软组织损伤。对于复发性脱位，逮捕测试（前部不稳定性）和窦沟征（下部不稳定性）是确定未来脱位倾向的有用方法。

脱位有三种主要类型：前，后和下。

前
在95％以上的肩关节脱位中，肱骨向前移位。[6]在大多数情况下，肱骨头在喙突过程中休息，称为亚喙突脱位。也可能发生下关节盂，锁骨下和非常罕见的胸内或腹膜后脱位[7]。

前脱位通常是由直接吹到或落在伸展的手臂上引起的。患者通常将他/她的手臂向外旋转并略微外展。

Hill-Sachs病变是关节脱位时关节盂边缘留下的肱骨头的影响。[5] Hill-Sachs畸形发生在35-40％的前脱位。当手臂处于内部旋转状态时，可以在正面X射线上看到它们。[8] Bankart病变是关节盂唇的破坏，伴有或不伴有骨碎片的撕脱。

后部脱位并不常见，通常是由于电击或癫痫发作引起的肌肉收缩[5]。它们可能是由肩袖肌肉的力量不平衡引起的。患者通常表现为手臂内旋和内收，并且通过突出的喙突过程表现出前肩扁平化。
