温度变化的模式可能偶尔暗示诊断：

病理生理学

温度最终在下丘脑中受到调节。发烧的一个触发因素称为热原，会引起前列腺素E2（PGE2）的释放。 PGE2反过来作用于下丘脑，在体内产生全身反应，产生发热效应以匹配新的更高温度设定点。

在许多方面，下丘脑的作用类似于恒温器。当设定点升高时，主体通过主动产生的热量和保持热量来升高其温度。外周血管收缩既减少了皮肤的热量损失，又使人感到寒冷。去甲肾上腺素增加棕色脂肪组织的产热，并且通过颤抖的肌肉收缩提高了代谢率。如果这些措施不足以使大脑中的血液温度与下丘脑中的新设定点相匹配，则开始颤抖以便使用肌肉运动产生更多热量。当下丘脑设定点自发地或用药物回到基线时，这些过程（血管舒张，颤抖和不发热产生的结束）和出汗的反向用于将身体冷却到新的较低设置。

热原
热原是一种诱发发烧的物质。这些可以是身体内部（内源性）或外部（外源性）。存在于革兰氏阴性细菌细胞壁中的细菌物质脂多糖（LPS）是外源性热原的一个例子。致热原性可以变化：在极端的例子中，一些被称为超级抗原的细菌热原可以引起快速和危险的发烧。可通过过滤，蒸馏，色谱或灭活实现脱热。

内源性

PGE2发布

PGE2是发热反应的最终调节剂。身体的设定点温度将保持升高，直到PGE2不再存在。 PGE2通过前列腺素E受体3（EP3）作用于视前区（POA）的神经元。在POA EP3-表达神经元神经支配背内侧下丘脑（DMH），在延髓（RRPA）延髓缝苍白核，和下丘脑室旁核（PVN）。发送到DMH和rRPa的发热信号导致交感神经输出系统的刺激，这引起非颤抖的产热以产生体热和皮肤血管收缩以减少体表的热损失。据推测，从POA到PVN的神经支配通过涉及脑垂体和各种内分泌器官的途径介导发热的神经内分泌作用。

下丘脑
大脑最终通过自主神经系统或主要运动中心协调热效应机制以使其颤抖。这些可能是：

肌肉张力增加，颤抖和肾上腺素（肾上腺素）等激素增加产热量
预防热量流失，如血管收缩。
在婴儿中，自主神经系统也可以激活棕色脂肪组织以产生热量（非运动相关的产热，也称为非颤抖的产热）。心率加快和血管收缩导致发烧时血压升高。

用处
有支持和反对发烧有用的论据，这个问题是有争议的。有些研究使用温血脊椎动物，其中一些研究表明它们可以更快地从感染或因发烧引起的严重疾病中恢复过来。研究表明，当发烧时，细菌感染的死亡率降低。

理论上，发烧可以帮助主机防御。肯定有一些重要的免疫反应会被温度加速，一些严格温度偏好的病原体可能会受到阻碍。

管理
不一定要治疗发烧。大多数人在没有特别医疗的情虽然令人不快，但即使未经治疗，发烧也很少升至危险水平。在温度达到42°C（107.6°F）之前，通常不会对大脑造成伤害，并且未经治疗的发烧超过40.6°C（105°F）是罕见的。治疗败血症患者的发热并不影响结果。

保守措施
一些有限的证据支持使用不温水的海绵或沐浴狂热的儿童。使用风扇或空调可能会在一定程度上降低温度并增加舒适度。如果温度达到极高水平的高热，则需要积极冷却（通常通过在大部分身体上施加大量冰袋或直接浸入冰水中通过传导机械地产生）。一般来说，建议人们保持充足的水分。是否增加液体摄入可改善症状或缩短呼吸道疾病如普通感冒尚不清楚。

药物
降低发烧的药物称为退热药。解热布洛芬可有效减少儿童发烧。它比儿童中的对乙酰氨基酚（扑热息痛）更有效。布洛芬和对乙酰氨基酚可以安全地用于患有发烧的儿童。对于患有发烧的儿童，对乙酰氨基酚本身的功效受到质疑。布洛芬在患有发烧的儿童中也优于阿司匹林。此外，由于存在雷氏综合症的风险，不建议儿童和年轻人（16岁或19岁以下的人，具体取决于国家）使用阿司匹林。

同时使用对乙酰氨基酚和布洛芬两者或在两者之间交替使用比仅使用对乙酰氨基酚或布洛芬更有效减少发热。目前尚不清楚它是否会增加儿童的舒适度。对药物的反应或无反应不能预测儿童是否患有严重疾病。

流行病学
大约5％的急诊室人发烧。

历史
早在公元前460年至公元前370年就已知多种类型的发烧，当时希波克拉底正在从事医学治疗，包括由于疟疾（第三次或每2天，每季度或每3天）。此时也很清楚，发烧是疾病的症状，而不是疾病本身。

社会与文化
词源

发烧恐惧症

其他动物
