嗅觉系统或嗅觉是用于嗅觉（嗅觉）的感觉系统的一部分。大多数哺乳动物和爬行动物具有主要的嗅觉系统和辅助嗅觉系统。主要的嗅觉系统检测空气中的物质，而辅助系统则检测流体相位刺激。

嗅觉和味觉（味觉系统）通常被称为化学感应系统，因为它们都通过称为转导的过程向大脑提供有关物体化学成分的信息。

外围系统

当吸气（嗅觉）时，气味分子可进入外周通路并通过鼻孔到达鼻腔，当舌头在咀嚼或吞咽（后鼻嗅觉）时将气体推向鼻腔后部时，气味分子通过喉咙。在鼻腔内，腔壁上的粘液溶解了气味分子。粘液还包括嗅上皮，其含有粘膜，产生和储存粘液和嗅腺，分泌粘液中的代谢酶。

转导
嗅觉神经元是上皮细胞中的受体细胞，其检测溶解在粘液中的气味分子，并在称为感觉转导的过程中将关于气味的信息传递给大脑。嗅觉神经元具有纤毛（微小毛发），其含有与气味分子结合的蛋白质，导致电响应通过受体细胞传播到鼻腔后部的嗅觉神经纤维。

嗅觉神经和纤维将来自外周嗅觉系统的气味信息传递到大脑的中央嗅觉系统，该大脑中央嗅觉系统通过筛骨的筛状板与上皮分离。起源于上皮的嗅觉神经纤维穿过筛状板，将上皮连接到嗅球上的大脑边缘系统。

中央
主要的嗅球将脉冲传递给二瓣和簇状细胞，这有助于根据某些神经元簇发射的时间确定气味浓度（称为“定时代码”）。这些细胞还注意到高度相似的气味之间的差异，并使用该数据来帮助以后识别。细胞不同，二瓣具有低的发射率并且容易被相邻细胞抑制，而簇绒具有高的发射速率并且更难以抑制。

临床意义

嗅觉丧失被称为嗅觉丧失。嗅觉障碍可发生在两侧或一侧。

嗅觉问题可根据其故障分为不同类型。嗅觉功能障碍可以是完全（嗅觉不全），不完全（部分嗅觉减退，嗅觉减退或微小症），扭曲（嗅觉障碍），或者可以通过像幻影症这样的自发感觉来表征。尽管嗅觉系统功能正常，但无法识别气味被称为嗅觉失认症。高碘血症是一种罕见的疾病，以嗅觉异常升高为特征。与视力和听力一样，嗅觉问题可以是双侧或单侧意义，如果一个人的鼻子右侧而不是左侧有嗅觉障碍，则是单侧右侧嗅觉缺失。另一方面，如果它位于鼻子两侧，则称为双侧嗅觉缺失或全嗅觉。

对嗅球，管道和初级皮层（布罗德曼区域34）的破坏导致与破坏同侧的嗅觉丧失。此外，突的刺激性病变导致嗅觉幻觉。

嗅觉系统的损伤可能由创伤性脑损伤，癌症，感染，吸入有毒烟雾或神经退行性疾病如帕金森病和阿尔茨海默病引起。这些情况可引起嗅觉丧失。相反，最近的发现表明嗅觉功能障碍的分子方面可以被认为是淀粉样蛋白形成相关疾病的标志，并且甚至可能通过破坏多价金属离子转运和储存存在因果关系。医生可以通过刮伤和吸气卡向患者提供气味或让患者闭上眼睛并尝试识别常见的气味，如咖啡或薄荷糖，来检测嗅觉系统的损害。在确诊嗅觉系统有永久性损伤之前，医生必须排除其他抑制或消除“嗅觉”的疾病，如慢性感冒或鼻窦炎。

嗅觉功能障碍的常见原因：高龄，病毒感染，接触有毒化学物质，头部创伤和神经退行性疾病。

年龄
年龄是健康成人嗅觉衰退的最强烈原因，其影响甚至比吸烟更大。嗅觉功能的年龄相关变化通常不会引起注意，并且与听力和视觉不同，嗅觉能力很少在临床上进行测试。 65岁以下的人中有2％患有慢性嗅觉问题。这在65岁到80岁之间的人中大大增加，其中大约一半人经历了严重的嗅闻问题。然后对于80岁以上的成年人，这一数字上升到近75％。与年龄相关的嗅觉功能变化的基础包括筛板的闭合，以及嗅觉受体在整个生命过程中反复的病毒和其他感染的累积损伤。

病毒感染
永久性嗅觉减退和嗅觉丧失的最常见原因是上呼吸道感染。这种功能障碍随着时间的推移没有变化，有时不仅可以反映嗅上皮的损伤，还可以反映由于病毒侵入大脑而导致的中央嗅觉结构。这些与病毒有关的疾病包括普通感冒，肝炎，流感和流感样疾病，以及疱疹。大多数病毒感染是无法识别的，因为它们是如此温和或完全无症状。

接触有毒化学品
长期暴露于某些空气中的毒素，如除草剂，杀虫剂，溶剂和重金属（镉，铬，镍和锰），会改变嗅觉的能力。这些药物不仅会损伤嗅上皮，而且很可能通过嗅粘膜进入大脑。

头部外伤
创伤相关的嗅觉功能障碍取决于创伤的严重程度以及头部是否发生强烈的加速/减速。枕骨和侧面撞击对嗅觉系统造成的损害比正面撞击更大。

神经退行性疾病
神经学家观察到嗅觉功能障碍是几种神经退行性疾病如阿尔茨海默病和帕金森病的主要特征。大多数这些患者在测试之前都没有意识到嗅觉缺陷，其中85％至90％的早期患者显示中央气味处理结构中的活性降低。

其他影响嗅觉功能障碍的神经退行性疾病包括亨廷顿舞蹈病，多发梗塞性痴呆，肌萎缩侧索硬化和精神分裂症。与阿尔茨海默氏症或帕金森病相比，这些疾病对嗅觉系统的影响更为温和。此外，进行性核上性麻痹和帕金森病仅与轻微的嗅觉问题有关。这些研究结果表明嗅觉测试可能有助于诊断几种不同的神经退行性疾病。

具有良好遗传决定因素的神经退行性疾病也与嗅觉功能障碍有关。例如，在患有家族性帕金森病和患有唐氏综合症的患者中发现这种功能障碍。进一步的研究得出结论，嗅觉丧失可能与智力残疾有关，而不是任何阿尔茨海默病样病理。

亨廷顿氏病还与气味识别，检测，辨别和记忆方面的问题有关。一旦该病症的表型成分出现，该问题就很普遍，尽管不知道嗅觉丧失在表型表达之前有多远。
