胃灼热，也称为酸性消化不良，是中央胸部或上中央腹部的灼烧感。这种不适常常在胸部上升，并可能辐射到颈部，喉咙或下颌角。

胃灼热通常是由于胃酸（胃反流）反流进入食道，并且是胃食管反流病（GERD）的主要症状。在约0.6％的病例中，它是缺血性心脏病的症状。

定义
术语消化不良包括胃灼热以及许多其他症状。消化不良有时被定义为上腹疼痛和胃灼热的组合。胃灼热通常与胃食管反流病交替使用，而不仅仅是描述一个人胸部灼烧的症状。

鉴别诊断
心脏病发作的症状和食道症状可能非常相似，因为心脏和食道使用相同的神经供应。

由于被忽视的心脏病发作所固有的危险，因此应首先考虑患有不明原因胸痛的人的心脏病。与GERD相关的胸痛患者很难与心脏病引起的胸痛患者区分开来。每种情况都可以模仿对方的体征和症状。进一步的医学调查，例如成像，通常是必要的。

心脏
胃灼热的症状可能与作为急性心肌梗塞（心脏病发作）或心绞痛症状的疼痛相混淆。燃烧或消化不良样疼痛的描述会增加急性冠状动脉综合征的风险，但不会达到统计学上显着的水平。在一群患有GERD症状的医院中，0.6％可能是由于缺血性心脏病。

多达30％的接受心脏导管插入术的胸痛患者的发现没有考虑到他们的胸部不适，并且通常被定义为具有“非典型胸痛”或未确定来源的胸痛。根据基于动态pH和压力监测的若干研究中记录的数据，估计这些患者中有25％至50％具有异常GERD的证据。

胃食管反流病
胃食管反流病是胃灼热的最常见原因。在这种情况下，酸反流导致食道炎症。

功能性胃灼热
功能性胃灼热是不明原因的胃灼热。它与其他功能性胃肠道疾病如肠易激综合征有关，并且是质子泵抑制剂（PPI）治疗后缺乏改善的主要原因。然而，PPI仍然是主要治疗方法，约50％的人的反应率。根据罗马III标准，诊断是消除之一：1）烧伤胸骨后不适; 2）消除心脏病发作和GERD为原因; 3）没有食道动力障碍。在一项调查中发现22.3％的加拿大人存在。

诊断方法
胃灼热可能由多种情况引起，GERD的初步诊断基于其他体征和症状。由GERD引起的胸痛具有明显的“灼烧”感，在进食后或在夜间发生，并且当人躺下或弯腰时恶化。它在孕妇中也很常见，可能是由于大量食用食物，或含有某些香料，高脂肪含量或高酸含量的特定食物引起的。如果怀疑胸痛是胃灼热，患者可能会经历上GI系列以确认是否存在胃酸反流。进食或饮酒后伴有胃灼热或胸痛，并伴有吞咽困难，可能表明食管痉挛。

在施用粘性利多卡因和抗酸剂后5至10分钟缓解症状增加了对疼痛源于食道的怀疑。然而，这并不排除可能的心脏病因，因为使用抗酸剂可以改善因心脏病引起的10％的不适。

生化
食道pH监测：探头可以通过鼻子放入食道，记录下食道的酸度。由于酸度的某种程度的变化是正常的，并且小的反流事件相对常见，因此可以使用食道pH监测来实时记录回流。

机械
测压法：在此测试中，压力传感器（压力计）通过口腔进入食道，直接测量下食道括约肌的压力。

内窥镜检查：食管粘膜可以直接通过一个薄的，照明的管子和一个称为内窥镜的微型相机通过口腔来检查食道和胃。通过这种方式，可以检测到食道炎症的证据，并在必要时进行活组织检查。由于内窥镜检查允许医生在视觉上检查上消化道，因此该过程可以帮助识别可能未检测到的对管道的任何额外损伤。

活组织检查：取出食管组织的一小部分样本。然后研究它以检查炎症，癌症或其他问题。

治疗
碳酸钙等抗酸剂通常用于治疗眼前问题，进一步治疗取决于根本原因。诸如H2受体拮抗剂或质子泵抑制剂的药物对胃炎和GERD有效，GERD是胃灼热的两种最常见原因。如果存在幽门螺杆菌，则使用抗生素。

流行病学
大约42％的美国人口在某些时候患有胃灼热。
