肺栓塞（PE）是一种物质通过血液（栓塞）从身体的其他部位移出来阻断肺部动脉。[6] PE的症状可能包括呼吸短促，特别是呼吸时胸痛，以及咳血。[1]也可能存在腿部血栓的症状，例如红色，温暖，肿胀和疼痛的腿。[1] PE的体征包括血氧水平低，呼吸急促，心率加快，有时还有轻度发烧[10]。严重的病例可导致昏倒，异常低血压和猝死。[2]

体征和症状
肺栓塞的症状通常是突然发作，可能包括以下一种或多种：呼吸困难（呼吸短促），呼吸急促（快速呼吸），胸膜炎“胸膜炎”（呼吸加重），咳嗽和咯血（咳血。）[16]更严重的病例可能包括紫绀（蓝色变色，通常是嘴唇和手指），塌陷和循环不稳定等症状，因为通过肺部和进入心脏左侧的血流量减少。所有猝死病例中约有15％归因于PE。[2]虽然PE可能伴有晕厥，但不到1％的晕厥病例是由于PE引起的。[17]

在体检中，肺通常是正常的。有时，在受影响的肺部区域（主要是梗塞的PE）可能会听到胸膜摩擦。胸腔积液有时是渗出性的，可通过减少的叩诊音，可听的呼吸音和声带共振来检测。右心室的应变可被检测为左胸骨旁隆起，第二心音的大声肺部分和/或颈静脉压升高。[2]可能存在低烧，特别是如果伴有肺出血或梗塞。[18]

由于较小的肺栓塞倾向于在没有侧支循环的更多周边区域中停留，它们更可能引起肺梗塞和小的积液（两者都是疼痛的），但不是缺氧，呼吸困难或血流动力学不稳定，例如心动过速。倾向于居中的较大的PE通常会引起呼吸困难，缺氧，低血压，心率加快和昏厥，但由于侧支循环没有肺梗塞，因此通常无痛。对于伴有胸膜炎疼痛，呼吸困难和心动过速的PE的典型表现可能是由于大的碎片栓塞引起的大小PE。因此，经常错过小的PE，因为它们单独引起胸膜炎疼痛而没有任何其他发现，并且经常错过大的PE，因为它们是无痛的并且模仿经常导致ECG改变和肌钙蛋白和BNP水平小幅上升的其他病症。[19]

血栓形成的发展通常是由于一组名为Virchow三联征的原因（血流改变，血管壁因素和影响血液特性的因素）。通常，存在多个风险因素。

根本原因
在第一次PE之后，寻找次要原因通常是短暂的。只有当第二个PE发生时，特别是当仍然在抗凝治疗时发生这种情况时，才会进一步寻找潜在的病症。这将包括因子V Leiden突变，抗磷脂抗体，蛋白C和S以及抗凝血酶水平，以及后来的凝血酶原突变，MTHFR突变，因子VIII浓度和稀有遗传性凝血异常的测试（“血栓形成屏幕”）。[22]

诊断

为了诊断肺栓塞，建议对临床标准进行审查，以确定是否需要进行检测。[23]风险低，年龄小于50岁，心率低于每分钟100次心跳，室内空气氧气水平超过94％，过去四周无腿部肿胀，咳血，手术或创伤的人，以前的血凝块或雌激素的使用，通常不需要进一步检测。[24]

如果有疑虑，接下来进行测试，以确定是否有可能通过成像确认诊断，然后进行成像，如果其他测试显示有可能进行PE诊断。[23] [25] [26]

PE的诊断主要基于经验证的临床标准与选择性检测相结合，因为典型的临床表现（呼吸急促，胸痛）无法与胸痛和呼吸短促的其他原因明确区分。执行医学成像的决定是基于临床推理，即体格检查的病史，症状和发现，然后评估临床概率。[2]

概率测试
预测临床概率的最常用方法Wells评分是临床预测规则，其使用因多个版本可用而变得复杂。 1995年，Philip Steven Wells最初开发了一种预测规则（基于文献检索），根据临床标准预测PE的可能性。[27]预测规则在1998年进行了修订[28]。在Wells等人的验证过程中简化后，该预测规则得到了进一步修订。在2000年。[29]在2000年的出版物中，威尔斯提出了两种不同的评分系统，使用截止值为2或4的预测规则相同。[29] 2001年，威尔斯发表的结果使用更为保守的截止值2来创建三个类别。[30]另一个版本，“修改后的扩展版本”，使用最近的2的截止值，但包括Wells的初步研究[27] [28]的发现。[31]最近，一项进一步的研究回归到韦尔斯早先使用4分的临界点[29]，只创造了两个类别。[32]

PE还有其他预测规则，例如日内瓦规则。更重要的是，任何规则的使用都与复发性血栓栓塞的减少有关。[33]

肺栓塞排除标准
肺栓塞排除标准（PERC）有助于评估疑似肺栓塞的人，但不太可能。与Wells评分和日内瓦评分不同，这些临床预测规则旨在对疑似PE患者进行分层，PERC规则旨在排除人们在将医生分为低风险类别时患PE的风险。

没有任何这些标准的低风险类别的人可能不会进行PE的进一步测试：低氧饱和度 -  SaO2 <95％，单侧腿部肿胀，咳血，先前DVT或PE，近期手术或创伤，年龄> 50，激素使用，心率快。这一决定背后的理由是，进一步的检测（特别是胸部的CT血管造影）可能比PE的风险造成更大的伤害（来自辐射暴露和对比染料）。[38] PERC规则的敏感性为97.4％，特异性为21.9％，假阴性率为1.0％（16/1666）。[39]

验血
对于低或中度怀疑PE的人，正常的D-二聚体水平（在血液测试中显示）足以排除血栓性PE的可能性，血栓栓塞事件的三个月风险为0.14％。[40] D-二聚体是高度敏感的，但不是特异性的（特异性约为50％）。换句话说，阳性D-二聚体与PE不同义，但阴性D-二聚体具有良好的确定性，表明不存在PE。[41]典型的截止值为500μg/ L，尽管这取决于测定。[42]然而，在50岁以上的人群中，建议将截止值改为人的年龄乘以10μg/ L（考虑已使用的化验），因为它减少了错误阳性测试的数量，而没有遗漏任何额外的PE案例。[24] [42] [43]

成像

CT肺动脉造影是大多数人推荐的一线诊断成像检查。[46]

从历史上看，诊断的黄金标准是肺血管造影，但随着非侵入性技术的增加，这已被废弃。[47]腿的超声可以确认PE的存在，但不能排除它。[48]

多排CT（MDCT）机器中可用检测器行数的快速变化阻碍了CT肺动脉造影的准确性评估。[49]根据一项队列研究，单层螺旋CT可能有助于诊断疑似肺栓塞患者的检测结果。[50]在这项研究中，敏感性为69％，特异性为84％。本研究中检出率为32％，阳性预测值为67.0％，阴性预测值为85.2％。然而，由于CT扫描是肺栓塞患者的最终诊断工具，因此该研究的结果可能由于可能的并入偏差而存在偏差。作者指出，阴性单层CT扫描不足以排除肺栓塞本身。一项单独的研究，使用4片和16片扫描仪的混合物报告了83％的敏感性和96％的特异性，这意味着如果在成像中没有看到它是排除肺栓塞的良好测试，并且它如果看到肺栓塞，则非常擅长确认肺栓塞。该研究指出，当临床概率与成像结果不一致时，还需要进行额外的检测。[51]与VQ扫描相比，CTPA不逊色于VQ扫描，并且识别出更多的栓子（不一定能改善结果）。[52]

通气/灌注扫描

低概率诊断测试/非诊断测试
经常进行的测试对PE不敏感，但可以诊断。

胸部X光通常在呼吸急促的人身上进行，以帮助排除其他原因，如充血性心力衰竭和肋骨骨折。 PE中的胸部X射线很少正常[56]，但通常缺乏表明PE诊断的迹象（例如，Westermark征，Hampton驼峰）。
腿部超声检查，也称为腿部多普勒，寻找深静脉血栓形成（DVT）。如在腿部超声检查中所示，DVT的存在本身足以保证抗凝，而不需要V / Q或螺旋CT扫描（因为DVT和PE之间的强关联）。这可能是怀孕期间的有效方法，其中其他方式会增加未出生婴儿出生缺陷的风险。然而，阴性扫描不排除PE，并且如果母亲被认为具有肺栓塞的高风险，则可能需要低辐射剂量扫描。因此，腿部超声检查的主要用途是那些临床症状提示深静脉血栓形成的患者。[55]

心电图

心电图的主要用途是排除胸痛的其他原因。[57]通常对患有胸痛的人进行心电图（ECG）以快速诊断心肌梗塞（心脏病发作），这是胸痛患者的重要鉴别诊断。虽然PE可能会发生某些心电图改变，但没有一个特定程度足以确认或足够敏感以排除诊断。[57]在大型PE的情况下，心电图可能显示出右心应变或急性肺心病的迹象 - 经典迹象是导联I中的大S波，导联III中的大Q波和导联III中的倒T波（S1Q3T3）在诊断中有12-50％的人发生，但在没有诊断的情况下也有12％发生。[58] [59]

偶尔会出现这种情况（多达20％的人发生），但也可能发生在其他急性肺部疾病中，因此诊断价值有限。心电图中最常见的体征是窦性心动过速，右轴偏离和右束支传导阻滞。[60]然而，窦性心动过速仅在8-69％的PE患者中发现[61]。

与肺栓塞相关的心电图检查结果可能表明预后较差，因为6例发现心电图上的RV应变（心率> 100次/分钟，S1Q3T3，导联V1-V4倒T波，aVR ST段抬高，完全右束支传导阻滞），和心房颤动）与循环休克和死亡风险增加有关。[62]

导联V1-3中倒T的病例被怀疑患有PE或下壁心肌梗塞。 PE病例在导联II和aVF中显示倒T波，但在II和aVF中，下壁心肌梗死病例未显示倒T波。[63]

超声心动图
在大块和亚大块PE中，心脏右侧功能障碍可见于超声心动图，表明肺动脉严重受阻，右心室（低压泵）无法与压力相匹配。一些研究（见下文）表明这一发现可能是溶栓的指征。并非所有患有（疑似）肺栓塞的人都需要超声心动图，但心肌肌钙蛋白或脑钠尿肽的升高可能表明心脏应变并需要超声心动图，[64]并且在预后中很重要[65]。

超声心动图右心室的特定外观称为McConnell征。这是中间自由壁的运动不能的发现，但是顶点的正常运动。在右心室功能障碍的情况下，这种现象对急性肺栓塞的诊断敏感性为77％，特异性为94％[66]。

预防
进一步的信息：预防血栓形成
有风险因素的人可能会预防肺栓塞。住院的人可能会接受预防性药物治疗，包括普通肝素，低分子肝素（LMWH）或磺达肝癸钠，以及抗血栓形成的长袜，以减少腿部可能移位并迁移到肺部的DVT风险。[68 ]

在先前的PE患者完成华法林治疗后，长期服用阿司匹林有助于预防复发。[69]

治疗
抗凝治疗是主要的治疗方法。急性地，可能需要支持性治疗，例如氧气或镇痛。人们经常在治疗的早期阶段住院，并且在INR达到治疗水平之前往往会继续接受住院治疗。然而，越来越多的低风险病例在家中以DVT治疗中常见的方式进行管理。[69] [70]支持一种方法与另一种方法的证据很薄弱。[71]

抗凝
通常，抗凝治疗是主要的治疗方法。最初施用未分级肝素（UFH），低分子量肝素（LMWH）或磺达肝素，同时开始华法林，醋硝香豆素或苯丙香豆素治疗（这可能需要数天，通常在患者住院期间）。根据Cochrane协作网对随机对照试验的系统评价，与UFH相比，LMWH可减少肺栓塞患者的出血。[72]根据同一综述，与肝素相比，LMWH降低了复发性血栓并发症的发生率并减少了血栓的大小。用LMWH治疗的患者和用普通肝素治疗的患者的总死亡率没有差异。

华法林治疗通常需要频繁调整剂量并监测国际标准化比率（INR）。在PE中，2.0和3.0之间的INR通常被认为是理想的。如果在华法林治疗下发生另一次PE发作，则INR窗口可以增加到例如2.5-3.5（除非有禁忌症）或抗凝可改为不同的抗凝剂，例如LMWH。[引证需要]

对于有潜在恶性肿瘤的患者，使用LMWH治疗的疗法优于华法林治疗;它会持续六个月，此时应决定是否需要持续治疗。[73]

同样，孕妇通常在分娩后至少六周内服用低分子量肝素，以避免华法林的已知致畸作用，特别是在怀孕的早期阶段。[55] [74]

华法林治疗通常持续3-6个月，如果之前存在DVT或PE，则“终身”，或者没有通常的危险因素。治疗结束时异常的D-二聚体水平可能表明首次无端肺栓塞患者需要继续治疗[75]。对于那些体重较小的PE（称为亚段PE），抗凝作用的效果尚不清楚，因为截至2014年尚未对其进行适当研究。[76]

溶栓
导致血流动力学不稳定的大量PE（休克和/或低血压，定义为收缩压<90 mmHg或40 mmHg的压降，如果不是由新发的心律失常，血容量不足或败血症引起的> 15分钟）是一种指征用于溶栓，用药物酶促破坏凝块。在这种情况下，对于那些没有禁忌症的人来说，它是最好的治疗方法，并得到临床指南的支持。[26] [73] [77]对于心脏骤停患者，也推荐使用已知的PE。[78]

导管溶栓（CDT）是一种新技术，对于大量PE而言相对安全有效。这涉及通过将导管放入腹股沟中的静脉并通过使用荧光透视成像引导其穿过静脉直到其位于肺循环中的PE旁边来进入静脉系统。通过导管释放破坏血凝块的药物，使其最高浓度直接靠近肺栓塞。 CDT由介入放射科医生或血管外科医生进行，在提供CDT的医疗中心，CDT可作为一线治疗提供。[79] 正在研究基于导管的超声辅助溶栓。[80]

在非大规模PE中使用溶栓治疗仍有争议。[81] [82] 有些人发现，这种治疗可以降低死亡风险并增加出血风险，包括颅内出血。[83] 其他人发现死亡风险没有下降。[82]

一旦安全开始使用抗凝治疗，应立即切除下腔静脉滤器。[73]虽然现代过滤器是可以检索的，但并发症可能会阻止一些过滤器被移除。永久性地将过滤器留在体内的长期安全性尚不清楚。[84]

手术
急性肺栓塞（肺血栓切除术）的外科治疗并不常见，并且由于长期预后不良而大部分被放弃。然而，最近，它已经通过手术技术的修订而复苏，并被认为有益于某些人。[85]导致肺动脉高压的慢性肺栓塞（称为慢性血栓栓塞性高血压）用称为肺血栓动脉内膜切除术的外科手术治疗。

流行病学
每年在美国有超过60万人患有肺栓塞。[7]它导致美国每年有50,000 [7]到200,000人死亡。[9]住院患者的风险约为1％。[86]在过去的25年中，美国的致命肺栓塞率从6％下降到2％。[9]

预测

有几种标记用于风险分层，这些也是不良结果的独立预测因子。这些包括低血压，心源性休克，晕厥，右心功能障碍的证据和心肌酶升高[26]。包括S1Q3T3在内的一些心电图改变也与较差的短期预后相关[20]。还有其他与患者相关的因素，如COPD和慢性心力衰竭，也被认为也在预后中发挥作用。[26]

预后取决于受影响的肺量和其他医疗条件的共存;慢性肺部栓塞可导致肺动脉高压。在大量PE后，如果患者要生存，必须以某种方式解决栓子。在血栓性PE中，血凝块可以通过纤维蛋白溶解分解，或者可以组织和再通，以便通过凝块形成新的通道。 PE后第一天或第二天血流量恢复得最快。[87]此后改善进展缓慢，一些赤字可能是永久性的。关于小的亚段PE是否需要治疗存在争议[88]，并且有证据表明，亚段PE患者可能在没有治疗的情况下表现良好。[51] [89]

一旦停止抗凝治疗，致命性肺栓塞的风险为每年0.5％。[90]
