缺血性坏死（AVN），也称为骨坏死或骨梗塞，是由于血液供应中断导致的骨组织死亡。[1]早期可能没有任何症状。[1]可能会逐渐出现关节疼痛，这可能会限制移动的能力。[1]并发症可能包括骨折或附近关节面的塌陷。[1]

体征和症状
在许多情况下，关节中的疼痛和不适随着时间的推移而增加。虽然它可以影响任何骨骼，但大约一半的病例显示出多个损伤部位。[5] 缺血性坏死主要影响肩部，膝部和髋部的关节。经典部位是：股骨头，距骨颈和舟状骨腰部。

缺血性坏死最常见于长骨的末端，例如股骨（从膝关节延伸到髋关节的骨）。其他常见部位包括肱骨（上臂骨），[6] [7]膝盖，[8] [9]肩，[6] [7]脚踝和下颌。[10]

原因
主要危险因素是骨折，关节脱位，酒精中毒和高剂量类固醇的使用。[1]其他风险因素包括放射治疗，化疗和器官移植。[1]骨坏死还与癌症，狼疮，镰状细胞病，HIV感染，戈谢病和沉箱病有关。[1]这种情况也可能没有任何明确的原因。[1]

双膦酸盐与下颌骨骨坏死有关。[11]长期，反复接触高压（如商业和军事潜水员所经历的）与AVN有关，尽管这种关系尚未得到充分了解。

病理生理学
造血细胞对低氧最敏感，并且在减少或去除血液供应后首先死亡，通常在12小时内。[2]实验证据表明骨细胞（骨细胞，破骨细胞，成骨细胞等）在12-48小时内死亡，骨髓脂肪细胞在5天内死亡。[2]

再灌注时，骨的修复分两个阶段进行。首先，血管生成和来自邻近活骨组织的未分化间充质细胞的运动生长到死骨髓空间，以及降解死细胞和脂肪碎片的巨噬细胞的进入。[2]其次，间充质细胞存在细胞分化为成骨细胞或成纤维细胞[2]。在有利条件下，剩余的无机矿物体积构成了建立新的全功能骨组织的框架。[2]

在早期阶段，骨骼闪烁扫描和MRI是首选的诊断工具。[12] [13]

早期缺血性坏死的X射线图像通常看起来正常。在后期阶段，由于附近的活骨再次被反应性充血引起再吸收，因此它看起来相对更加无线电。[2] 坏死骨本身没有显示增加的放射学不透明度，因为死骨不能进行骨吸收，这是由活的破骨细胞进行的。[2] 晚期射线照相标志还包括软骨下骨（新月征）崩溃后的射线可透区域和由髓质梗塞后骨髓脂肪的皂化和钙化引起的无线电密度的环形区域。

椎间真空裂隙征（白色箭头）是缺血性坏死的标志。椎体压缩性骨折后椎体缺血性坏死称为Kümmel病[14]。

类型
当AVN影响舟状骨时，它被称为Preiser病。 AVN的另一种命名形式是Köhler病，其影响足部的舟骨，/主要是儿童。

治疗
可以使用多种方法来治疗[5]，最常见的是全髋关节置换术（THR）。然而，THR具有许多缺点，包括长恢复时间和短寿命跨度（髋关节）。 THR是老年人群的有效治疗手段;然而，在年轻人中，他们可能会在一个人的生命结束之前磨损。

金属表面重修等其他技术可能并不适用于所有缺血性坏死的病例;它的适用性取决于股骨头的损伤程度。[15]降低骨质破坏率的双膦酸盐可以防止因AVN引起的塌陷（特别是髋部）[16]。

核心减压
其他治疗包括核心减压，其中通过在骨中钻孔来减轻内部骨压，并且植入活骨片和刺激新血管生长的电子装置;和游离血管腓骨移植物（FVFG），其中一部分腓骨及其血液供应被移除并移植到股骨头中。[17] 2012年Cochrane系统评价指出，与单独的物理治疗相比，髋关节核心减压和参与物理治疗的人之间没有明显的改善。需要更多的研究来研究髋部核心减压对镰状细胞病患者的有效性。[18]

通过在核心减压后将骨髓中的有核细胞移植到缺血性坏死病灶中可能会阻止疾病的进展，尽管需要进一步的研究来建立这种技术。[19] [20]

预测
由缺血性坏死引起的残疾量取决于骨骼的哪个部分受到影响，涉及的区域有多大，以及骨骼如何有效地重建自身。骨重建过程发生在受伤后以及正常生长过程中。[15]通常情况下，骨骼不断破裂，重建旧骨骼会被吸收并被新骨骼取代。这个过程使骨骼保持强壮，并有助于维持矿物质的平衡。[15]然而，在缺血性坏死的过程中，愈合过程通常是无效的，并且骨组织比身体可以修复它们更快地分解。如果不及时治疗，疾病就会进展，骨折，[21]并且关节面破裂，导致疼痛和关节炎。[1]

流行病学
缺血性坏死通常影响30至50岁的人;每年约有10,000至20,000人在美国发生股骨头缺血性坏死。当它发生在股骨头的儿童身上时，它被称为Legg-Calvé-Perthes综合征。[22]
