髓母细胞瘤是儿童中最常见的原发性脑癌类型。它起源于大脑的背部和底部部分，头骨底部，小脑或后颅窝。[1]

大脑分为两个主要部分，顶部是较大的大脑，后面是较小的小脑。它们被称为天幕的膜隔开。因此，起源于小脑或小脑幕下方周围区域的肿瘤称为幕下肿瘤。

历史上，髓母细胞瘤被归类为原始神经外胚层肿瘤（PNET），但现在已知髓内细胞瘤与幕上PNET不同，它们不再被认为是类似的实体。[2]

髓母细胞瘤是侵入性的，快速生长的肿瘤，与大多数脑肿瘤不同，它通过脑脊液传播并经常转移到沿着脑和脊髓表面的不同位置。一直到脊髓底部马尾的转移被称为“跌落转移”。

所有年龄组和组织学随访的累积相对存活率分别为5年，10年和20年的60％，52％和47％，儿童表现优于成人[3]。

体征和症状
症状和体征主要是由于第四脑室阻塞引起的颅内压继发增加，肿瘤通常在诊断前存在1至5个月。孩子通常会变得无精打采，反复发作呕吐和早晨头痛，这可能导致胃肠道疾病或偏头痛的误诊。[4] 不久之后，孩子会出现蹒跚步态，躯干性共济失调，频繁跌倒，复视，视神经乳头水肿和第六脑神经麻痹。位置性头晕和眼球震颤也很常见，并且可能存在面部感觉丧失或运动无力。去大脑攻击出现在疾病晚期。

对身体其他部位的神经外转移是罕见的，并且当它发生时，它处于复发的环境中，更常见于常规化疗之前的时代。

发病
髓母细胞瘤通常位于第四脑室附近，脑干和小脑之间。具有相似外观和特征的肿瘤起源于大脑的其他部位，但它们与髓母细胞瘤不同。[5]

尽管髓母细胞瘤在其发育的最早阶段被认为起源于未成熟或胚胎细胞，但起源细胞依赖于髓母细胞瘤的亚组。 WNT肿瘤起源于脑干的下部菱形唇，而SHH肿瘤起源于外部颗粒层。[引证需要]

目前，髓母细胞瘤被认为是由小脑干细胞产生的，这些小脑干细胞已经被阻止分裂并分化成它们的正常细胞类型。这解释了活组织检查中观察到的组织学变异。血管周假性假体和荷马赖特玫瑰花结假体形成是髓母细胞瘤的高度特征，并且在多达一半的病例中可见。[6]可以看到经典的莲座丛，中央管腔周围有肿瘤细胞。[7]

在过去，髓母细胞瘤是使用组织学进行分类的，但最近的整合基因组研究表明，髓母细胞瘤由四种不同的分子和临床变体组成，称为WNT /β-catenin，Sonic Hedgehog，Group 3和Group 4 [8]。在这些亚组中，WNT患者预后良好，3组患者预后不良。此外，亚组特异性选择性剪接进一步证实了不同亚组的存在，并突出了亚组之间的转录异质性。[9] Sonic Hedgehog通路的扩增是最具特征的亚组，25％的人类肿瘤在Patched，Sufu（Fused Homolog抑制因子），Smoothened或其他基因中具有突变[10] [11]。髓母细胞瘤也见于Gorlin综合征以及Turcot综合征。在具有髓母细胞瘤的个体中鉴定出基因CTNNB1，PTCH1，MLL2，SMARCA4，DDX3X，CTDNEP1，KDM6A和TBR1中的复发突变[12]。在一些髓母细胞瘤中被破坏的其他途径包括MYC，Notch，BMP和TGF-β信号传导途径。[10] [11] [4] [13] [14] [15] [16] [17]

诊断
肿瘤在T1和T2加权MRI上是独特的，具有异质增强和邻近并延伸到第四脑室的典型位置。在组织学上，肿瘤是实心的，粉红色 - 灰色，并且被很好地限制。肿瘤是非常细胞的，具有高的有丝分裂活性，很少的细胞质，并且倾向于形成簇和玫瑰花结。

髓母细胞瘤的正确诊断可能需要排除非典型的畸胎样横纹肌瘤[18]。

治疗
治疗始于最大程度的手术切除肿瘤。向整个神经轴和化疗中添加辐射可以增加无病生存率。一些证据表明，质子束照射可减少辐射对耳蜗和心血管区域的影响，并减少颅内照射的认知迟发效应。[19] [20]这种组合可以允许超过80％的病例存活5年。结缔组织形成等促纤维化特征的存在提供了更好的预后。如果孩子小于3岁，切除程度不足，或者发生任何脑脊液，脊柱，幕上或全身扩散，预后更差。放疗和化疗后痴呆是治疗后2至4年出现的常见结果。放射治疗的副作用可能包括认知障碍，精神疾病，骨骼发育迟缓，听力丧失和内分泌紊乱[1] [4] [13]。颅内压增高可以通过皮质类固醇或脑室腹腔分流来控制。

化疗
化疗通常用作治疗的一部分。然而，截至2013年，收益证据尚不清楚。[21]使用一些不同的髓母细胞瘤化疗方案;大多数涉及洛莫司汀，顺铂，卡铂，长春新碱或环磷酰胺的组合。在年龄较小的患者（年龄小于3-4岁）中，化疗可能会延迟，或在某些情况下甚至可能消除对放疗的需求。然而，化疗和放疗往往具有长期毒性作用，包括身体和认知发育的延迟，第二次癌症的风险增加以及心脏病风险增加[22] [23]。

成果
基于阵列的260髓母细胞瘤的核型分析基于细胞遗传学特征导致以下临床亚组：[24]

预后不良：获得6q或MYC或MYCN的扩增
中级：增益为17q或i（17q），不增加6q或放大MYC或MYCN
预后良好：6q和17q平衡或6q缺失

生存
所有年龄组和组织学随访的累积相对存活率分别为5年，10年和20年的60％，52％和47％。被诊断患有髓母细胞瘤或PNET的患者死亡的可能性是一般人群的匹配成员的50倍。最近的基于人群的（SEER）5年相对存活率总体上为69％：儿童（1-9岁）为72％，成人（20岁以上）为67％。儿童的20年生存率为51％。儿童和成人的生存状况不同，仅在确诊后第四年（控制了背景死亡率增加后），成人的表现比儿童更差。在第四年之前，生存概率几乎相同。[3]标准治疗的长期后遗症包括下丘脑 - 垂体和甲状腺功能障碍和智力障碍。这些疗法产生的荷尔蒙和智力缺陷导致幸存者严重受损。[26]

流行病学
髓母细胞瘤对每年每百万人的影响不到两个人，对儿童的影响是成人的10倍。[27]髓母细胞瘤是儿童毛细胞星形细胞瘤后第二常见的脑肿瘤[28]，是儿童中最常见的恶性脑肿瘤，占新诊断脑肿瘤的14.5％[29]。在成人中，髓母细胞瘤是罕见的，包含不到2％的CNS恶性肿瘤。[30]

儿童髓鞘细胞瘤的新病例率在男性（62％）中高于女性（38％），这一特征在成人中未见[27] [31]。髓母细胞瘤和其他PNET在年幼的儿童中比年龄较大的儿童更为普遍。大约40％的髓母细胞瘤患者在5岁之前被诊断出来，31％在5到9岁之间，18.3％在10到14岁之间，12.7％在15到19岁之间。 [32]。

研究模型
