外痔
在约40％的病理性痔疮患者中，没有明显的症状。内部和外部痔疮的表现可能不同；但是，许多人可能两者兼而有之。足以引起贫血的出血很少见，威胁生命的出血更为罕见。许多人在面对问题时会感到尴尬，并且常常仅在案件进展时才寻求医疗服务。

外部
如果不进行血栓形成，则外部痔疮可能引起的问题很少。但是，当血栓形成时，痔疮可能会非常痛苦。但是，这种疼痛通常会在两到三天内消失。但是，肿胀可能需要几个星期才能消失。愈合后可能会残留皮肤标签。如果痔疮很大并且引起卫生问题，它们可能会刺激周围的皮肤，从而使肛门周围发痒。

内部

原因
症状性痔疮的确切病因尚不清楚。人们认为有许多因素在起作用，包括不规则的排便习惯（便秘或腹泻），缺乏运动，营养因素（低纤维饮食），腹腔内压力升高（长时间的紧张，腹水，腹腔内肿块）。（或怀孕），遗传因素，痔静脉内无瓣膜以及衰老。被认为会增加风险的其他因素包括肥胖，久坐，慢性咳嗽和骨盆底功能障碍。排便时蹲下也可能增加严重痔疮的风险。但是，这些关联的证据很少。

在怀孕期间，胎儿对腹部的压力和荷尔蒙的变化会导致痔疮血管增大。婴儿的出生也导致腹腔内压力增加。孕妇很少需要手术治疗，因为通常在分娩后症状会缓解。

病理生理学
痔疮垫是正常人体解剖学的一部分，并且只有在出现异常变化时才成为病理疾病。正常的肛管中存在三个主要垫子。这些通常位于左侧，右侧和右侧后部位置。它们既不由动脉也不由静脉组成，而是由被称为正弦曲线，结缔组织和平滑肌的血管组成。：175正弦曲线不像静脉那样在其壁上具有肌肉组织。这组血管被称为痔丛。

痔疮垫对节制很重要。它们在静止时占肛门闭合压力的15–20％，并在粪便通过期间保护肛门内括约肌和肛门外肌。当人躺下时，腹腔内压力会增加，痔疮垫的大小会增加，有助于维持肛门闭合。当这些血管结构向下滑动或静脉压力过分升高时，会导致痔疮症状。内外肛门括约肌压力升高也可能与痔疮症状有关。两种类型的痔疮发生：上痔丛的内部和下痔丛的外部。果胶线将两个区域分开。

诊断
痔疮通常通过身体检查来诊断。肛门和周围区域的视觉检查可以诊断出外部或脱垂的痔疮。可以进行直肠检查以发现可能的直肠肿瘤，息肉，前列腺肿大或脓肿。尽管大多数内部痔疮与疼痛无关，但由于疼痛，如果没有适当的镇静剂，可能无法进行此项检查。视觉确认内部痔疮可能需要进行镜检，插入空心管装置，一端附有灯。痔疮的两种类型是外部的和内部的。这些通过相对于果胶线的位置来区分。某些人可能同时具有两种症状的症状。如果存在疼痛，则该病更有可能是肛裂或外痔而不是内痔。

内部

微分
许多肛门直肠问题，包括裂痕，瘘管，脓肿，结肠直肠癌，直肠静脉曲张和瘙痒，都有类似的症状，可能被错误地称为痔疮。直肠出血，结肠炎（包括炎症性肠病，憩室病和血管增生）也可能引起直肠出血。如果存在贫血，应考虑其他潜在原因。

产生肛门肿块的其他情况包括皮肤标签，肛门疣，直肠脱垂，息肉和肛门乳头肿大。由于门脉高压（门静脉系统中的血压升高）引起的肛门直肠静脉曲张可能与痔疮相似，但情况不同。门静脉高压并不增加痔疮的风险。

预防
建议采取多种预防措施，包括避免尝试排便时劳损，通过进食高纤维饮食，喝大量液体或服用纤维补充剂来避免便秘和腹泻，并进行充分的运动。还建议您花更少的时间尝试排便，避免在马桶上看书，为超重人士减轻体重并避免繁重的举重。

管理
保守
保守治疗通常包括富含膳食纤维的食物，摄入保持水合作用的口服液，非甾体抗炎药，坐浴和休息。已显示出增加的纤维摄入量可以改善结局，并且可以通过饮食改变或摄入纤维补充剂来实现。然而，缺乏证据表明在治疗的任何时候都可以从坐浴中受益。如果使用它们，一次应限制在15分钟之内。：182也建议减少在马桶上花费的时间并且不要拉紧。

尽管许多外用药物和栓剂可用于治疗痔疮，但几乎没有证据支持其使用。因此，美国结肠和直肠外科医生协会不推荐使用它们。含类固醇的药物不应使用超过14天，因为它们可能会导致皮肤变薄。大多数药剂包括活性成分的组合。这些可以包括隔离霜，例如凡士林或氧化锌，止痛剂例如利多卡因，和血管收缩剂例如肾上腺素。有些含有秘鲁的苦瓜，某些人可能对此过敏。

类黄酮具有令人怀疑的益处，具有潜在的副作用。怀孕后症状通常会缓解；因此主动治疗通常会延迟到分娩后才进行。证据不支持使用传统中草药治疗。

几个专业协会极力建议使用静脉注射植物药治疗I至II级痔疮的症状，尽管这些药物截至2013年在美国和德国尚未获得批准，并且在西班牙仅限于治疗慢性静脉疾病。

程序
可以执行许多基于诊室的程序。虽然通常是安全的，但可能会发生罕见的严重副作用，如肛周脓毒症。

通常建议将橡皮筋结扎术作为I至III级疾病患者的一线治疗。该过程是在果胶酸盐线上方至少1 cm的内部痔疮上施加橡皮筋以切断其血液供应。在5至7天内，枯萎的痔疮脱落。如果绑带太靠近齿状线，那么紧接着会导致剧烈疼痛。已发现治愈率约为87％，并发症发生率高达3％。
硬化疗法包括将硬化剂（例如苯酚）注射入痔疮。这导致静脉壁塌陷，痔疮萎缩。治疗四年后的成功率约为70％。
多种烧灼方法已被证明对痔疮有效，但通常仅在其他方法失败时才使用。可以使用电灼，红外辐射，激光手术或冷冻手术来完成此过程。红外线烧灼可能是I级或II级疾病的一种选择。在患有III级或IV级疾病的患者中，复发率很高。

手术
如果保守治疗和简单操作失败，则可以使用多种外科技术。由于直肠与供应膀胱的神经非常接近，因此所有外科手术治疗都会引起一定程度的并发症，包括出血，感染，肛门狭窄和尿光潴留。而且，发生粪便失禁的风险很小，尤其是液体的失禁风险，据报道在0％至28％之间。粘膜外翻是痔切除术后可能发生的另一种情况（通常与肛门狭窄一起发生）。这是肛门粘膜从肛门外翻的地方，类似于非常轻度的直肠脱垂。

切除性痔切除术是仅在严重情况下才使用的痔疮的手术切除术。它伴有严重的术后疼痛，通常需要两到四个星期才能恢复。但是，与橡胶带结扎相比，III级痔疮患者的长期获益更大。如果血栓形成的外部痔疮在24-72小时内进行，则推荐治疗。但是，支持这一点的证据很少。手术后的甘油三硝酸酯软膏有助于缓解疼痛和愈合。
多普勒引导下经肛门痔除血是一种微创治疗，使用超声多普勒精确定位动脉血流。然后将这些动脉“打结”，并将脱垂的组织缝合回到其正常位置。与痔切除术相比，它的复发率略高，但并发症较少。
吻合钉痔切除术，也称为吻合钉痔切除术，涉及切除许多异常扩大的痔疮组织，然后将剩余的痔疮组织重新定位回其正常的解剖位置。与完全清除痔疮相比，它通常不那么痛苦，并且愈合更快。但是，有症状的痔疮复发的机会比常规的痔切除术要大，因此通常仅建议用于II级或III级疾病。

流行病学
难以确定痔疮的常见程度，因为许多患有这种疾病的人没有去看保健医生。但是，据认为有症状的痔疮在其一生中的某些时候会影响至少50％的美国人口，并且在任何给定时间都会影响大约5％的人口。男女患病的几率大致相同，发病高峰在45至65岁之间。它们在高加索人和较高社会经济地位的人中更常见。

尽管有些人可能会反复出现症状性发作，但长期结果通常是好的。只有一小部分人最终需要手术。

值得注意的情况
