溃疡性结肠炎（UC）是一种长期病，可导致结肠和直肠发炎和溃疡。活动性疾病的主要症状是腹痛和腹泻与血液混合。体重减轻，发烧和贫血也可能发生。通常，症状会缓慢出现，范围从轻度到严重。症状通常间歇发作，发作之间无症状。并发症可能包括巨结肠，眼睛，关节或肝脏发炎以及结肠癌。

胃肠道

轻度疾病与每天少于四个大便相关，有或没有血液，没有全身毒性迹象，并且正常的红细胞沉降率（ESR）或C反应蛋白（CRP）。可能会出现轻度的腹痛或绞痛。患有UC的人可能认为他们实际上在经历里急后重的便秘，这是一种不断的感觉，需要排空肠胃，伴随着不自愿的劳累，疼痛和绞痛，几乎没有粪便排出。直肠疼痛不常见。
中度疾病与每天四次以上大便有关，但毒性反应极小。人们可能会出现贫血（不需要输血），中度腹痛和38至39摄氏度（100至102华氏度）的低烧。
严重的疾病与每天超过六个大便血便或可观察到的大量明显的大便血运动有关，并伴随有发烧，心动过速，贫血或ESR或CRP升高的毒性证据。
暴发性疾病与每天超过10次排便，持续出血，毒性，腹部压痛和腹胀，输血需求以及结肠扩张（扩张）有关。这类人的炎症可能不仅限于粘膜层，还导致结肠运动受损，并导致有毒的巨结肠。如果累及浆膜，可能会发生结肠穿孔。除非得到治疗，否则暴发性疾病将很快导致死亡。

肠外功能

原因
尚无导致UC的直接原因，但许多可能的因素如遗传和压力都起作用。

遗传因素
可以基于以下假设来推测导致UC的遗传因素：

家庭溃疡性结肠炎的聚集
同卵双胞胎一致率为10％，同卵双胞胎一致率为3％
种族差异案例数
遗传标记和连锁
基因组的十二个区域可能与UC连接，按照发现顺序包括16、12、6、14、5、19、1和3号染色体，但是这些位点均未始终显示在断层，表明该疾病受多种基因影响。例如，染色体带1p36就是这样一种与炎症性肠病有关的区域。

一些假定的区域编码转运蛋白，例如OCTN1和OCTN2。其他潜在区域涉及细胞支架蛋白，例如MAGUK家族。人类白细胞抗原关联甚至可能起作用。实际上，在第6号染色体上的这种联系可能是最令人信服和最一致的遗传候选物。

神经内脏和皮肤遗传性卟啉症已被记录为多种自身免疫性疾病，包括UC，克罗恩氏病，乳糜泻，疱疹样皮炎，糖尿病，系统性和盘状狼疮，类风湿性关节炎，强直性脊柱炎，硬皮病，Sjgren综合征和巩膜炎。对于患有自身免疫性疾病的家庭，医师应高度警惕卟啉症，必须谨慎使用潜在的卟啉原性药物，包括柳氮磺吡啶。

环境因素
对于导致溃疡性结肠炎的发病机理的环境因素提出了许多假设。它们包括：

饮食：由于结肠暴露于许多可能引起炎症的饮食物质中，因此饮食因素被认为在溃疡性结肠炎和克罗恩氏病的发病机理中均起着作用。很少有研究调查这种关联。一项研究表明，精制糖与溃疡性结肠炎的感染人数没有关联。大量摄入不饱和脂肪和维生素B6可能会增加发生溃疡性结肠炎的风险。其他可能影响疾病发展和/或复发的饮食因素包括肉蛋白和酒精饮料。具体而言，已经调查了硫与溃疡性结肠炎的病因有关，但这是有争议的。已经在患有UC和该疾病动物模型的人中研究了硫限制饮食。硫作为病因的理论除了与饮食有关外，还与肠道菌群和粘膜硫化物解毒有关。

自身免疫病
溃疡性结肠炎是一种以T细胞浸润结肠为特征的自身免疫性疾病。与克罗恩病不同，克罗恩病会影响结肠外的胃肠道区域，溃疡性结肠炎通常累及直肠，并局限于结肠，偶有回肠受累。这种所谓的“反流性回肠炎”可能发生在10-20％的胰腺炎患者中，据认为其临床意义不大。溃疡性结肠炎也可能与合并症有关，合并症在消化系统以外的身体许多区域产生症状。外科手术切除大肠通常可以治愈该病。

溃疡性结肠炎患者中还原硫酸盐细菌的水平往往较高，这可能表明肠道中的硫化氢水平较高。另一种理论认为该疾病的症状可能是由于硫化氢对肠壁细胞的毒性作用引起的。

已提出由鸟分枝杆菌副结核亚种感染是溃疡性结肠炎和克罗恩氏病的最终原因。

在某些溃疡性结肠炎患者中，发现减少结肠硫酸盐的细菌数量增加，导致有毒气体硫化氢的浓度更高。人结肠粘膜由结肠上皮屏障和固有层中的免疫细胞维持（见肠粘膜屏障）。 N-丁酸，一种短链脂肪酸，通过β氧化途径被氧化成二氧化碳和酮体。已经显示丁酸N有助于向该上皮屏障提供营养。研究表明，硫化氢通过中断短链乙酰辅酶A脱氢酶（短链乙酰辅酶A脱氢酶）而在破坏该β-氧化途径中起作用。此外，已经提出吸烟在溃疡性结肠炎中的保护益处是由于来自香烟烟雾中的氰化氢与硫化氢反应生成无毒的异硫氰酸盐，从而抑制了硫化物中断该途径。一项无关的研究表明，红肉和酒精中所含的硫可能导致缓解的人复发的风险增加。

溃疡性结肠炎患者通常会出现直肠出血，腹泻，里急后重（迫切地希望排空肠道，但粪便很少）和下腹部疼痛。临床表现的疾病严重程度对于确定适当的治疗方法很重要。患有轻度活动疾病的人每天排便次数少于4次，并且没有毒性迹象。 UC中度严重者的排便频率更高，并伴有出血。大约70％的溃疡性结肠炎患者会出现中度活动性疾病。患有严重活动性疾病的人会出现发烧，心动过速和贫血的中毒迹象。患有暴发性或中毒性结肠炎或中毒性巨结肠的人一天通常排便10次以上，持续流血，腹胀和压痛，并有水肿的放射学证据，在某些情况下还包括肠扩张。这些人通常需要立即进行结肠切除术，因为10％的人在手术时已经穿孔了结肠。

诊断

溃疡性结肠炎的初步诊断检查包括以下内容：

进行全血细胞计数以检查贫血；血小板增多，血小板增多，偶尔可见

进行肝功能检查以筛查胆管受累：原发性硬化性胆管炎。

内窥镜

可能合并的浅表溃疡
假性息肉。
溃疡性结肠炎通常是从直肠持续发生的，几乎普遍累及了直肠。肛周疾病很少见。内窥镜检查受累的程度从直肠的直肠炎或发炎到左侧结肠炎再到胰腺炎，后者是涉及升结肠的炎症。

组织学
进行粘膜活检可明确诊断UC，并将其与克罗恩氏病区分开，克罗恩氏病的临床处理方法不同。微生物样品通常在内窥镜检查时采集。溃疡性结肠炎的病理学通常包括隐窝结构变形，隐窝炎症（隐窝炎），坦率的隐窝脓肿，固有层出血或炎性细胞。在临床情况不清楚的情况下，组织形态分析通常在确定诊断进而确定治疗方面起关键作用。相比之下，活检分析可能不确定，因此该疾病的临床进展必须为其治疗提供依据。

恶性肿瘤–癌症可能表现为结肠炎的急性发作，反之亦然。重要的是要排除恶性肿瘤，尤其是当结肠炎对治疗无效时。
模仿溃疡性结肠炎症状的最常见疾病是克罗恩氏病，因为两者都是炎症性肠病，可以以类似症状影响结肠。区分这些疾病很重要，因为它们的疗程和治疗方法可能有所不同。但是，在某些情况下，可能无法分辨出差异，在这种情况下，该疾病被归类为不确定性结肠炎。

管理

溃疡性结肠炎的标准治疗取决于受累程度和疾病严重程度。目的是首先通过药物诱导缓解，然后再施用维持药物以预防复发。诱导缓解和维持缓解的概念非常重要。用于诱导和维持缓解的药物有些重叠，但是治疗方法不同。医师首先直接治疗以诱导缓解，这包括缓解症状和结肠内膜的粘膜愈合，然后进行长期治疗以维持缓解并预防并发症。急性重症溃疡性结肠炎需要住院治疗，排除感染和使用糖皮质激素。

对于疾病的急性期，可能建议低纤维饮食。

用药
溃疡性结肠炎可以用多种药物治疗，包括5-ASA药物，如柳氮磺吡啶和美沙拉嗪。也可以使用皮质类固醇，例如泼尼松，因为它们具有免疫抑制作用和短期愈合特性，但是由于其风险大于其益处，因此不能长期用于治疗。仅当人们无法通过5-ASA和皮质类固醇获得缓解时，才给予诸如硫唑嘌呤之类的免疫抑制药物以及诸如英夫利昔单抗和阿达木单抗之类的生物制剂。在中度或重度疾病患者中，建议使用英夫利昔单抗或维多珠单抗。由于其可能的危险因素，此类治疗的使用较少，包括但不限于青少年和成人患癌症，结核病以及新发或恶化的心力衰竭的风险增加（这些副作用很少见）。

2013年1月，布地奈德的一种制剂被美国食品和药物管理局（FDA）批准用于治疗活动性溃疡性结肠炎。2018年，托法替尼被批准用于美国的中度至重度活动性溃疡性结肠炎的治疗，这是第一种口服药物表示在这种情况下可以长期使用。甲氨蝶呤的证据并未显示出在溃疡性结肠炎患者中缓解症状的益处。标签外使用诸如环孢菌素和他克莫司的药物已显示出一些好处。非索非那定是一种用于治疗过敏的抗组胺药，在一些研究中已显示出其在联合疗法中的前景。合适的是，非索非那定的低胃肠道吸收（或高吸收的药物胃肠道分泌物）导致炎症部位的浓度更高。因此，该药物可通过累及胃肠肥大细胞局部减少组胺分泌并减轻炎症。

氨基水杨酸酯
柳氮磺胺吡啶在轻度至中度溃疡性结肠炎的治疗中已有50多年的历史了。 1977年，研究表明5-氨基水杨酸（5-ASA，美沙拉嗪/美沙拉敏）是柳氮磺吡啶的治疗活性成分。已经开发了许多5-ASA药物，其目的是将活性化合物递送至大肠以维持治疗功效，但是减少了与柳氮磺吡啶类中的磺胺吡啶部分有关的副作用。口服5-ASA药物在轻度至中度溃疡性结肠炎的诱导和维持缓解方面特别有效。 5-ASA的直肠栓剂，泡沫剂或液体灌肠剂可用于影响直肠，乙状结肠或降结肠的结肠炎，并且已证明特别有效，尤其是与口服治疗联用时。

生物制剂
诸如TNF抑制剂英夫利昔单抗，阿达木单抗和戈利木单抗的生物疗法通常用于治疗对皮质类固醇无反应的UC患者。 Tofacitinib，vedolizumab和etrolizumab还可在UC中产生良好的临床缓解和缓解率。通常，这些药物仅在其他选择都用尽的情况下才使用（即该人已接受高剂量皮质类固醇和免疫调节剂（如硫唑嘌呤和美沙拉嗪）且反应不佳）。

尼古丁
与克罗恩氏病不同，溃疡性结肠炎比不吸烟者更容易感染吸烟者。使用经皮尼古丁贴剂的研究显示出临床和组织学方面的改善。

在英国进行的一项双盲，安慰剂对照研究中，使用尼古丁贴剂及其标准治疗的48.6％UC患者显示出症状的完全缓解。在美国进行的另一项随机，双盲，安慰剂对照，单中心临床试验表明，使用该贴剂的人中有39％表现出显著改善，而使用安慰剂的人中这一比例为9％。与不使用尼古丁的标准治疗相似，使用不添加其他标准治疗方法（如美沙拉嗪）的经皮尼古丁贴剂的复发发生率相似。

铁补充
胃肠道血液的逐渐流失以及慢性炎症通常会导致贫血，专业指南建议在活动性疾病中每三个月重复一次血液检查，并每年定期在静止性疾病中定期进行血液检查以监测贫血。适当的疾病控制通常可以改善慢性疾病的贫血，但是缺铁性贫血应使用补铁治疗。给予治疗的形式取决于贫血的严重程度和所遵循的指南。一些人建议首先使用肠胃外铁剂，因为人们对其反应更快，与胃肠道副作用较少相关，并且与依从性问题无关。其他人则要求首先使用口服铁剂，因为人们最终会做出反应，许多人会耐受副作用。所有指南均建议在严重贫血（血红蛋白水平低于100 g / L）的情况下应使用肠胃外铁剂。

与克罗恩氏病不同，溃疡性结肠炎的胃肠道问题通常可以通过手术切除大肠来治愈，尽管肠外症状可能会持续存在。如果发生以下情况，则必须执行此步骤：出血，坦率穿孔或有文件证明或高度怀疑的癌。还建议对患有严重结肠炎或中毒性巨结肠的人进行手术。那些患有致残症状且对药物无反应的人不妨考虑一下手术是否会改善生活质量。

溃疡性结肠炎会影响肠道外的身体许多部位。在极少数情况下，该疾病的肠道外表现可能需要切除结肠。

影响大多数大肠的溃疡性结肠炎的另一种手术选择称为回肠肛门袋手术。这是一个两到三步的过程，其中除去了直肠残端和肛门之外的大肠，并进行了临时的回肠造口术。手术的下一部分可以分一步或两步完成，通常距每一次先前的手术间隔六至十二个月。

在手术的下一步中，用小肠自身的小肠制成一个内部小袋，然后将该小袋内部钩回到直肠残端，这样该人就可以再次拥有一个功能正常的肠系统，所有内部都如此。暂时回肠造口术此时可以逆转，以便使该人因肠功能而内化，或者在该过程的另一步骤中，可以将袋和直肠残端吻合术留在该人体内，以便在该人仍处于静止状态的情况下恢复一段时间将回肠造口术用于肠功能。然后，在随后的手术中，回肠造口术被逆转并且该人再次具有内在的肠功能。

髂肛门袋是一种永久治疗，因为它是永久性的改变，并且可能会引起新的并发症。通常需要进一步的检查。

白细胞单采
一种称为粒细胞和单核细胞吸附性单采血液分离的白细胞单采血液分离仍需要进行大规模试验来确定其是否有效。小型试验的结果暂定为阳性。

细菌再定植
在许多随机临床试验中，益生菌已显示出有助于治疗溃疡性结肠炎的潜力。已显示特定类型的益生菌（如大肠杆菌Nissle）可在某些人中诱导长达一年的缓解。据说具有类似作用的另一种益生菌是嗜酸乳杆菌。据说益生菌通过平息一些引起该疾病的正在进行的炎症来起作用，从而使人体动员树突状细胞，也就是信使免疫细胞。这些细胞然后能够产生其他T细胞，这些T细胞通过重新平衡系统性炎症进一步帮助恢复肠道平衡。
粪便微生物群移植涉及通过粪便灌肠注入人类益生菌。与艰难梭菌感染相比，溃疡性结肠炎通常需要更长时间的细菌疗法治疗才能成功，这可能是由于治愈溃疡上皮所需的时间所致。溃疡性结肠炎的反应可能非常有利，一项研究报告67.7％的患者完全缓解。其他研究发现使用粪便微生物群移植有好处。这表明溃疡性结肠炎的病因可能是先前仍被未知的病原体感染。这种最初的感染自然会自行消退，但会以某种方式导致结肠细菌菌群失衡，从而导致炎症循环，可以通过用健康肠道的细菌“重新定殖”结肠来打破炎症循环。有几起报告的案例，这些人一直处于缓解状态长达13年之久。

替代药物
约有21％的炎症性肠病患者使用替代疗法。多种饮食疗法显示出希望，但在推荐之前需要进一步研究。

根据体外研究，动物研究和初步的人体研究，褪黑激素可能是有益的。
膳食纤维，即不易消化的植物物质，已被推荐用于维持肠功能数十年。特别值得注意的是来自芸苔的纤维，它似乎含有能够在煮熟之前在整个人体消化道中逆转溃疡的可溶性成分。
鱼油和鱼油中的二十碳五烯酸（EPA）抑制白三烯活性，后者可能是炎症的关键因素。作为IBD疗法，尚无确凿的研究支持和推荐剂量。但是对于其他情况（最常见的是心脏疾病），建议每天使用180至1500 mg /天的EPA。鱼油还含有维生素D，其中许多患有IBD的人都缺乏维生素D。
短链脂肪酸（丁酸酯）灌肠。结肠中的上皮细胞利用肠内容物中的丁酸盐作为能源。可用的丁酸酯量向直肠减少。已建议在小肠中丁酸水平不足是该病的一个促成因素。这可以通过丁酸盐灌肠来解决。但是结果不是结论性的。
溃疡性结肠炎患者使用草药。含有巯基的化合物可能对溃疡性结肠炎有影响（在类似的假设下，柳氮磺胺吡啶的磺胺部分除具有活性的5-ASA成分外，还可能具有活性）。一项随机对照试验评估了非处方药S-甲基甲硫氨酸，并发现当与口服柳氮磺胺吡啶联用时，复发率显著降低。
蠕虫病疗法是利用肠道寄生线虫治疗溃疡性结肠炎，并且是基于卫生学假设的前提。研究表明，蠕虫在阻断小鼠化学诱导的结肠炎方面比日常皮质类固醇激素改善并且更有效，并且一项针对特发性慢性腹泻（与人类溃疡性结肠炎类似的疾病）的猕猴有意感染蠕虫的试验还可以缓解症状。五分之四的动物被治疗。猪Trichuris suis ova在人体中的一项随机对照试验发现该疗法安全有效，并且正在进行进一步的人体试验。
姜黄素（姜黄）疗法，与服用美沙拉敏或柳氮磺胺吡啶合用，对于保持静止期溃疡性结肠炎患者的缓解可能是有效和安全的。姜黄素单独治疗对静态溃疡性结肠炎的作用尚不清楚。

进展或缓解

溃疡性结肠炎患者通常具有间歇性病程，疾病不活跃的时期与疾病的“耀斑”交替出现。直肠炎或左侧结肠炎患者的病程通常更为良性：只有15％的患者在疾病近端进展，如果不进行任何治疗，则高达20％的患者可以持续缓解。疾病较严重的人较难维持缓解，但缓解率与疾病的严重程度无关。

大肠癌
如果累及脾曲张，则溃疡性结肠炎患者十年后大肠癌的风险会显著增加。反流性回肠炎患者可能会增加结直肠癌的风险。仅患有直肠炎或直肠乙状结肠炎的人通常没有增加的风险。建议人们进行八年的疾病活动后，以一到两年的间隔对结肠镜检查进行随机活检，以检查是否异常增生。

原发性硬化性胆管炎
溃疡性结肠炎与原发性硬化性胆管炎（PSC）密切相关，后者是大小胆管的进行性炎性疾病。多达5％的溃疡性结肠炎患者可能会发展为原发性硬化性胆管炎。

死亡
研究还没有显示出溃疡性结肠炎患者的整体死亡风险与背景人群的死亡风险没有任何差异。死亡原因分布可能与背景人群的死亡原因分布不同。认为该疾病主要影响生活质量，而不影响寿命。

其他长期特征
在慢性溃疡性结肠炎中可以看到的变化包括颗粒度，粘膜血管形态的丧失，胃泌素的丧失，回盲瓣膜的脱落，粘膜桥接，狭窄和假性息肉。

流行病学
UC和克罗恩氏病的地理分布在全球范围内相似，在加拿大，新西兰和英国，一年中UC的新病例数最多。它最常见于15至25岁之间。第二个发病高峰是生命的第六个十年。通常，与欧洲和美国的南部地区相比，北部地区的房价更高。

与克罗恩氏病一样，阿什肯纳兹犹太人的UC发生率较高，而其他犹太裔，非犹太白种人，非洲人，西班牙裔和亚洲人的UC发病率则逐渐降低。 20岁之前因阑尾炎和当前吸烟而进行的阑尾切除术可预防UC的发展（尽管以前吸烟会增加患此病的风险。）

美国
截至2004年，美国每年新发UC病例数为每十万分之2.2至14.3。在美国，受影响的人数是每10万人37至246之间。

加拿大
在加拿大，从1998年到2000年，每年的新病例数是每100,000人口12.9例，即4,500例新病例。受影响的人数估计为每10万人211人或104,000。

英国
在英国，每年每10万人中有10人新患此病，而受影响的人数是每100,000人243人。在英国，大约有146,000人被诊断患有UC。

研究
爱荷华州的一项随机对照试验显示，使用鞭毛猪鞭毛虫进行蠕虫病治疗对溃疡性结肠炎患者有好处。该疗法检验了卫生学假说，该假说认为发达国家的人结肠中没有蠕虫，可能导致炎症。蠕虫病治疗和粪便微生物群移植均可在患病区域诱发特征性的Th2白细胞反应，这是出乎意料的，因为溃疡性结肠炎被认为与Th2的过度产生有关。

Alicaforsen是第一代反义寡聚脱氧核苷酸，设计用于通过Watson-Crick碱基对相互作用与人ICAM-1信使RNA特异性结合，以抑制ICAM-1的表达。 ICAM-1传播炎症反应，促进白细胞（白细胞）外渗和激活进入发炎的组织。在溃疡性结肠炎患者的发炎的肠粘膜中观察到ICAM-1表达的增加，其中ICAM-1的过量产生与疾病活动相关。这表明ICAM-1是溃疡性结肠炎的潜在治疗靶标。

内腔中存在的革兰氏阳性细菌可能与溃疡性结肠炎的复发时间延长有关。

正在开发的一系列药物希望通过选择性靶向炎症信号级联反应中称为KCa3.1的离子通道来破坏炎症过程。在大鼠和小鼠的临床前研究中，对KCa3.1的抑制与柳氮磺胺吡啶一样有效地破坏了Th1细胞因子IL-2和TNF-α的产生，并降低了结肠炎症。

在溃疡性结肠炎患者的临床缓解中，在结肠粘膜中有嗜中性粒细胞胞外陷阱和由此导致的胞外基质降解的报道，这表明病原学涉及先天免疫系统。
