胃肠炎，也称为传染性腹泻和胃病，是胃肠道（胃和小肠）的炎症。症状可能包括腹泻，呕吐和腹痛。发烧，精神不振和脱水也可能发生。这通常持续不到两周。尽管它被错误地称为“胃流感”，但它与流感无关。

预防包括用肥皂洗手，喝清水，适当处理人类废物和母乳喂养婴儿，而不要使用配方奶。建议使用轮状病毒疫苗预防儿童。治疗涉及获取足够的液体。对于轻度或中度病例，通常可以通过喝口服补液（水，盐和糖的混合物）来实现。对于那些采用母乳喂养的人，建议继续母乳喂养。对于更严重的情况，可能需要静脉输液。也可以通过鼻胃管注入液体。建议儿童补充锌。通常不需要抗生素。但是，对于发烧和血性腹泻的幼儿，建议使用抗生素。

体征和症状

肠胃炎通常涉及腹泻和呕吐。有时，只有一个或另一个存在。这可能伴有腹部绞痛。感染病原体感染后通常在12-72小时内开始出现体征和症状。如果是由于病毒引起的，该病通常会在一周内解决。一些病毒感染还涉及发烧，疲劳，头痛和肌肉疼痛。如果大便带血，则病因不太可能是病毒，更可能是细菌。一些细菌感染会导致严重的腹痛，并可能持续数周。

感染了轮状病毒的儿童通常会在三到八天内完全康复。然而，在贫穷国家中，严重感染的治疗常常无法达到，持续性腹泻很常见。脱水是腹泻的常见并发症。如果按下时皮肤颜色和位置缓慢恢复，则可能会发现儿童严重脱水。这被称为“毛细血管补充过长”和“皮肤膨松不良”。呼吸异常是严重脱水的另一个迹象。在卫生条件差和营养不良的地区通常会出现反复感染。可能会导致发育迟缓和长期认知迟缓。

弯曲杆菌属感染后，有1％的人会发生反应性关节炎。格林-巴雷综合征的发生率为0.1％。溶血性尿毒症综合征（HUS）可能是由于感染志贺毒素的大肠杆菌或志贺氏菌引起的。 HUS导致血小板计数低，肾脏功能差和红细胞计数低（由于其分解）。儿童比成年人更容易感染HUS。一些病毒感染可能会导致婴儿良性癫痫发作。

原因
病毒（特别是轮状病毒）以及大肠杆菌和弯曲杆菌是引起肠胃炎的主要原因。但是，还有许多其他可导致该综合征的感染因子，包括寄生虫和真菌。有时会看到非感染性原因，但与病毒或细菌性原因相比，它们的可能性较小。由于缺乏免疫力，儿童感染的风险更高。儿童的风险也较高，因为他们不太可能养成良好的卫生习惯。生活在难以获得水和肥皂的地区的儿童尤其容易受到伤害。

病毒式
已知轮状病毒，诺如病毒，腺病毒和星状病毒会引起病毒性肠胃炎。轮状病毒是儿童肠胃炎的最常见原因，在发达国家和发展中国家中，轮状病毒的发病率均相似。在小儿年龄段，病毒引起约70％的感染性腹泻发作。由于获得性免疫，轮状病毒在成年人中不太常见。诺如病毒是所有病例中约18％的原因。

诺如病毒是美国成年人肠胃炎的主要原因，引起超过90％的暴发。这些局部流行病通常发生在一群人在彼此身体上非常接近的地方度过的时光，例如在游轮，医院和饭店里。即使腹泻结束，人们也可能仍然具有传染性。诺如病毒是造成儿童病例约10％的原因。

细菌

沙门氏菌血清型鼠伤寒沙门氏菌（ATCC 14028），用显微镜在1000倍的放大倍数下观察，然后进行革兰氏染色。
在发达国家，空肠弯曲菌是细菌性肠胃炎的主要原因，其中一半病例与家禽接触有关。在儿童中，细菌是约15％的原因，最常见的类型是大肠杆菌，沙门氏菌，志贺氏菌和弯曲杆菌。如果食物被细菌污染并在室温下放置数小时，细菌就会繁殖并增加食用食物者的感染风险。通常与疾病相关的一些食物包括生或未煮熟的肉，家禽，海鲜和鸡蛋；生芽未经巴氏消毒的牛奶和软奶酪；以及水果和蔬菜汁。在发展中国家，特别是撒哈拉以南非洲和亚洲，霍乱是引起胃肠炎的常见原因。这种感染通常是由受污染的水或食物传播的。

产毒素的艰难梭菌是引起腹泻的重要原因，而腹泻在老年人中更为常见。婴儿可以携带这些细菌而不会出现症状。这是住院患者的腹泻的常见原因，并经常与抗生素的使用有关。使用抗生素的人也可能发生金黄色葡萄球菌感染性腹泻。急性“旅行者腹泻”通常是细菌性肠胃炎的一种，而持久性形式通常是寄生性的。暴露于许多生物体（包括艰难梭菌，沙门氏菌和弯曲杆菌）后，抑酸药物似乎会增加发生重大感染的风险。服用质子泵抑制剂的人比使用H2拮抗剂的人更大。

寄生
多种寄生虫可引起肠胃炎。贾第鞭毛虫最常见，但溶血性变形虫，隐孢子虫和其他物种也有牵连。作为一个整体，这些药物占儿童病例的约10％。贾第鞭毛虫在发展中国家更为普遍，但是这种疾病几乎可以在任何地方发生。它更普遍地发生在去过高流行地区的人，参加日托的儿童，与男人发生性关系的男人以及灾难之后。

传播
传播可能是由于饮用受污染的水或人们共享个人物品而发生的。在雨季，水质通常会恶化，而此时暴发更为普遍。在四个季节的地区，冬季感染更为常见。在世界范围内，用未经适当消毒的奶瓶喂养婴儿是一个重要原因。传播率也与卫生状况差（特别是在儿童中），拥挤的家庭以及营养状况差的家庭有关。具有免疫力的成年人可能仍携带某些生物体而没有症状。因此，成年人可以成为某些疾病的天然库。尽管某些病原体（例如志贺氏菌）仅发生在灵长类动物中，但其他病原体（例如贾第虫）则可能存在于多种动物中。

非传染性
胃肠道炎症有多种非感染性原因。一些较常见的包括药物（如NSAIDs），某些食物，如乳糖（对于不耐受者）和面筋（对于患有乳糜泻的人）。克罗恩氏病也是（通常是严重的）胃肠炎的非感染源。毒素继发的疾病也可能发生。与恶心，呕吐和腹泻相关的一些与食物相关的状况包括：因食用被污染的掠食性鱼类而引起的中美洲虎中毒，由于食用某些类型的变质鱼而引起的类群，因食用河豚而引起的河豚毒素中毒以及肉毒杆菌中毒通常是由于食物保存不当造成的。

在美国，从2006年到2011年，急诊科非传染性胃肠炎的使用率下降了30％。在急诊科看到的最常见的二十种情况中，非传染性胃肠炎的发病率在该时期内下降幅度最大。

病理生理学

肠胃炎是由于肠道感染引起的。
肠胃炎定义为由于小肠或大肠发炎引起的呕吐或腹泻，通常是由于感染引起的。小肠的变化通常是非炎症性的，而大肠的变化是炎症性的。引起感染所需的病原体数量从少至一种（对于隐孢子虫）到多至108种（对于霍乱弧菌）不等。

诊断
胃肠炎通常根据人的体征和症状进行临床诊断。通常不需要确定确切原因，因为它不会改变病情的管理。

但是，应该对那些粪便中有血液的人，那些可能接触过食物中毒的人以及最近去过发展中国家的人进行粪便培养。它也可能适合5岁以下的儿童，老年人以及免疫功能较差的儿童。也可以进行诊断测试以进行监视。由于低血糖发生在大约10％的婴幼儿中，因此建议对该人群进行血糖测量。当担心严重脱水时，也应检查电解质和肾功能。

脱水
确定患者是否脱水是评估的重要部分，脱水通常分为轻度（3–5％），中度（6–9％）和重度（≥10％）。在儿童中，中度或重度脱水的最准确体征是长时间的毛细血管补充，皮肤充沛的不适和呼吸异常。其他有用的发现（当组合使用时）包括眼睛凹陷，活动减少，没有眼泪和口干。正常的尿量和口服液摄入量可以放心。实验室测试在确定脱水程度方面几乎没有临床益处。因此，通常不需要使用尿液测试或超声检查。

鉴别诊断
需要排除的其他可能模仿肠胃炎的体征和症状的潜在原因包括阑尾炎，肠扭转，炎性肠病，尿路感染和糖尿病。还应考虑胰腺功能不全，短肠综合征，Whipple's病，腹腔疾病和泻药滥用。如果该人仅表现出呕吐或腹泻（而不是两者），则鉴别诊断可能会有些复杂。

在多达33％的病例中，阑尾炎可能伴有呕吐，腹痛和少量腹泻。这与胃肠炎典型的大量腹泻相反。儿童肺部或泌尿道感染也可能引起呕吐或腹泻。经典的糖尿病酮症酸中毒（DKA）表现为腹痛，恶心和呕吐，但无腹泻。一项研究发现，最初诊断为DKA的儿童中有17％患有肠胃炎。

预防

2000年7月至2009年6月美国监测周轮状病毒检测呈阳性结果的百分比。
生活方式
提供容易获得的未污染水和良好的卫生习惯对于降低感染率和临床上重要的胃肠炎至关重要。已发现，在发展中国家和发达国家，个人措施（例如用肥皂洗手）均可将肠胃炎的发病率降低多达30％。醇基凝胶也可能有效。应该避免被认为受到污染的食物或饮料。母乳喂养很重要，尤其是在卫生条件较差的地方，这通常对改善卫生状况也很重要。母乳可以减少感染的频率和持续时间。

预防接种
由于其有效性和安全性，世界卫生组织于2009年建议向全球所有儿童提供轮状病毒疫苗。现有两种商业轮状病毒疫苗，还有几种正在研发中。在非洲和亚洲，这些疫苗减轻了婴儿的严重疾病，实施了国家免疫计划的国家的疾病发生率和严重性有所下降。这种疫苗还可以通过减少循环感染的数量来预防未接种疫苗的儿童患病。自2000年以来，在美国实施了轮状病毒疫苗接种计划，从而使腹泻病例数大大减少了80％。应给6至15周龄的婴儿接种第一剂疫苗。口服霍乱疫苗在两年内被发现有效率为50-60％。

管理
胃肠炎通常是一种急性且自限性的疾病，不需要药物治疗。在轻度至中度脱水的患者中，首选的治疗方法是口服补液疗法（ORT）。对于有呕吐脱水危险的儿童，服用单剂量的抗呕吐药物甲氧氯普胺或恩丹西酮可能会有所帮助，而butylscopolamine可用于治疗腹痛。

补液
儿童和成人胃肠炎的主要治疗方法是补液。尽管如果意识水平降低或脱水严重，可能需要静脉输液，但最好通过饮用补液解决。饮用由复合碳水化合物制成的替代疗法产品（即由小麦或大米制成的替代疗法产品）可能优于基于简单糖的替代疗法产品。不建议5岁以下的儿童饮用特别富含简单糖的饮料，例如汽水和果汁，因为它们可能会加重腹泻。如果没有更具体的ORT制剂或该人不愿意饮用，可以使用白开水。如果需要，可以在幼儿中使用鼻胃管来输液。对于那些需要静脉输液的人，一到四个小时的时间通常就足够了。

饮食
建议继续以常规方式对母乳喂养的婴儿进行护理，并在用ORT补液后立即继续对配方奶喂养的婴儿继续其配方奶。通常不需要无乳糖或减少乳糖的配方。儿童应在腹泻期间继续常规饮食，但应避免高糖饮食。不再推荐使用BRAT饮食（香蕉，大米，苹果酱，烤面包和茶），因为它的营养成分不足，对正常喂养没有好处。

已经显示一些益生菌在减少疾病持续时间和减少大便次数方面是有益的。它们还可用于预防和治疗抗生素相关性腹泻。发酵乳制品（例如酸奶）同样有益。补充锌似乎对发展中国家儿童的腹泻治疗和预防均有效。

止吐药
止吐药可能有助于治疗儿童呕吐。恩丹西酮具有一定的实用性，单剂量可减少对静脉输液的需求，减少住院，并减少呕吐。甲氧氯普胺可能也有帮助。然而，使用恩丹西酮可能与儿童返院率增加有关。如果临床判断需要，可以口服恩丹西酮的静脉内制剂。二氢海藻酸钠在减少呕吐的同时，似乎没有明显的临床益处。

抗生素类
抗生素通常不用于肠胃炎，尽管有时如果症状特别严重，或者查出或怀疑是易感细菌引起的，则有时建议使用抗生素。如果要使用抗生素，则大环内酯类（例如阿奇霉素）优于氟喹诺酮，因为对后者的耐药率更高。伪膜性结肠炎通常是由抗生素引起的，可以通过中断病原体并用甲硝唑或万古霉素治疗来治疗。适于治疗的细菌和原生动物包括鼠伤寒沙门氏菌和贾第虫。对于那些患有贾第虫属或溶血性变形虫的人，建议使用替硝唑治疗，优于甲硝唑。世界卫生组织（世卫组织）建议在血痢和发烧的幼儿中使用抗生素。

抗运动剂
抗蠕动药在理论上有引起并发症的风险，尽管临床经验表明这种可能性不大，但对于血性腹泻或发烧并发腹泻的人，不建议使用这些药物。阿片类药物洛哌丁胺通常用于对症腹泻的对症治疗。不建议儿童使用洛哌丁胺，因为它可能会穿过未成熟的血脑屏障并引起毒性。亚水杨酸铋是三价铋和水杨酸的不溶性复合物，可用于轻度至中度的病例，但是水杨酸的毒性理论上是可能的。

流行病学

胃肠炎是美国每年370万人次就诊，法国300万人次就诊的主要原因。据信，在美国，整个胃肠炎每年造成的费用为230亿美元，仅轮状病毒引起的费用估计为每年10亿美元。

研究
有许多针对胃肠炎的疫苗正在开发中。例如，针对志贺氏菌和产肠毒素的大肠杆菌（ETEC）的疫苗是全世界胃肠炎的两个主要细菌病因。

其他动物
与人类一样，许多相同的因素也会在猫和狗中引起肠胃炎。最常见的生物是弯曲杆菌，艰难梭菌，产气荚膜梭菌和沙门氏菌。大量有毒植物也可能引起症状。

一些试剂对某些物种更具特异性。猪中传播性胃肠炎冠状病毒（TGEV）导致呕吐，腹泻和脱水。据信它是由野生鸟类引入猪中的，没有具体的治疗方法。它不能传播给人类。
