脑淀粉样血管病（CAA）是一种血管病，其中淀粉样β肽沉积在中枢神经系统和脑膜的中小血管壁上。有时使用术语“嗜刚果金”是因为淀粉样蛋白异常聚集的存在可以通过用刚果红染色后脑组织的显微镜检查来证明。淀粉样物质仅在大脑中发现，因此该疾病与其他形式的淀粉样变性无关。

体征和症状
由于这可能是由与阿尔茨海默氏症相关的淀粉样蛋白引起的，因此脑出血在诊断为阿尔茨海默氏病的人中更为常见。但是，它们也可能发生在没有痴呆病史的患者中。大脑内的出血通常仅限于特定的叶，这与由于高血压（出血性中风（或大脑内出血）的更常见原因）而发生的脑出血相比略有不同。

原因

星形胶质细胞
CAA被确定为偶发（通常发生在老年人口）或家族形式，例如佛兰德语，爱荷华州和荷兰语类型。在所有情况下，其定义都是由Aβ沉积在软脑膜和脑血管壁中引起的。已经观察到以佛兰德语类型出现的CAA与该谱系中观察到的大的密集核斑块相关。

零星CAA中Aβ沉积增加的原因仍不清楚，因为肽的产生增加和异常清除都被认为是潜在的原因。在正常生理状态下，Aβ可通过四种途径从大脑清除：（1）星形胶质细胞和小胶质细胞的内吞作用；（2）脑啡肽或胰岛素溶酶的酶促降解作用；（3）通过血脑屏障清除的作用；或（4）沿动脉周围排出空格。这些确定的清除途径中的每一种异常都与CAA相关。

在家族形式的CAA中，Aβ积累的原因可能是由于产量增加而不是清除率差所致。淀粉样蛋白前体蛋白（APP），早老素（PS）1和PS2基因的突变可导致APP裂解为Aβ的速率增加。还提出了一种免疫机制。

诊断

MRI在脑淀粉样血管病中显示低信号灶。常规梯度回波T2 *加权图像（左，TE = 20ms），磁化率加权图像（SWI）和SWI相位图像（分别为中心和右，TE = 40ms）在1.5 Tesla下。
CAA只能通过验尸来明确诊断。活检可以在诊断可能的病例中发挥作用。当没有可用的组织进行活检时，将使用波士顿标准从MRI或CT扫描数据确定可能的CAA病例。《波士顿标准》要求有多发性大叶或皮层出血的证据，以将患者标记为可能患有CAA。磁化加权成像已被提议作为识别CAA相关微出血的工具。

种类
它通常与淀粉样蛋白β相关。

目前正在进行研究以确定脑淀粉样血管病与摄入过量铝之间是否存在联系。

管理
脑淀粉样血管病的目的是治疗症状，因为目前尚无治愈方法。物理，职业和/或言语治疗可能有助于控制这种状况。
