细菌性阴道病（BV）是细菌过度生长引起的阴道疾病。常见症状包括白带增多，通常闻起来像鱼。放电通常为白色或灰色。可能会因排尿而燃烧。瘙痒不常见。有时可能没有任何症状。拥有BV会使包括HIV / AIDS在内的许多性传播感染的感染风险大约增加一倍。这也增加了孕妇早期分娩的风险。

通常用抗生素治疗，例如克林霉素或甲硝唑。这些药物也可用于妊娠中期或晚期。但是，这种病通常会在治疗后复发。益生菌可能有助于防止再次发生。目前尚不清楚使用益生菌或抗生素是否会影响妊娠结局。

体征和症状
常见症状包括白带增多，通常闻起来像鱼。放电物通常为白色或灰色。排尿时可能会燃烧。有时可能没有任何症状。

尽管有时会出现轻度的瘙痒，但分泌物会覆盖阴道壁，通常没有明显的刺激，疼痛或红斑（发红）。相比之下，正常的白带在整个月经周期中的稠度和量会有所不同，并且在排卵时最清澈-大约在月经开始前两周。一些从业者声称BV在几乎一半的患病女性中无症状，尽管其他人则认为这通常是误诊。

并发症
尽管以前仅被认为是令人讨厌的感染，但未经治疗的细菌性阴道病可能会导致对包括艾滋病毒在内的性传播感染和妊娠并发症的敏感性增加。

研究表明，患有细菌性阴道病（BV）的HIV感染妇女比没有BV的妇女更容易将HIV传播给性伴侣。 BV的诊断标准也与诱导HIV表达的女性生殖道因子有关。

有证据表明，BV与性传播感染（例如HIV / AIDS）的发生率增加之间存在关联。 BV与HIV释放量增加多达六倍有关。 BV是病毒脱落和2型单纯疱疹病毒感染的危险因素。 BV可能会增加人乳头瘤病毒（HPV）感染或激活的风险。

此外，既存的或后天的细菌性阴道病都可能增加怀孕并发症的风险，最明显的是早产或流产。 BV孕妇的绒毛膜羊膜炎，流产，早产，胎膜早破和产后子宫内膜炎的风险较高。接受体外受精治疗的BV妇女的着床率和早期流产率更高。

原因

健康的阴道微生物群由既不会引起症状或感染也不会对怀孕产生负面影响的物种组成。它主要由乳酸杆菌属所主导。 BV由阴道微生物群中的不平衡以及乳杆菌数量的减少来定义。尽管感染涉及多种细菌，但据信大多数感染始于阴道加德纳菌形成生物膜，从而使其他机会性细菌得以繁衍。

发生BV的主要风险之一是冲洗，这会改变阴道微生物群，并使女性更易患BV。出于这个原因和其他原因，美国卫生与公共服务部和各种医疗机构都强烈建议不要进行重复检查。

BV是盆腔炎，HIV，性传播感染（STIs），生殖和产科疾病或阴性结果的危险因素。不从事性活动的人有可能发展细菌性阴道病。

细菌性阴道病有时可能会影响更年期后的女性。同样，亚临床铁缺乏症可能与怀孕初期的细菌性阴道病有关。 2006年2月发表在《美国妇产科杂志》上的一项纵向研究表明，即使考虑到其他危险因素，心理社会压力与细菌性阴道病之间的联系仍然存在。接触杀精子剂nonoxynol-9不会影响细菌性阴道病的风险。

有女性伴侣会增加BV风险60％。与BV相关的细菌已从男性生殖器中分离出来。在感染女性的男性伴侣的阴茎，冠状沟和男性尿道中发现了BV菌群。尚未割礼的伴侣可以充当“水库”，增加性交后感染的可能性。 BV相关微生物群的另一种传播方式是通过皮肤向皮肤的性行为传递给女性性伴侣。 BV可通过会阴肠细菌从雌性和雄性生殖器的微生物群中传播。

诊断

用于检测阴道碱化作用的pH指示剂（此处显示约为pH 8），以及显微镜载玻片，用于显微镜检测线索细胞。
为了诊断细菌性阴道病，应从阴道内获取拭子。这些棉签应进行以下测试：

湿式安装时有典型的“腥”味。通过将少量氢氧化钾添加到含有白带的显微镜载玻片上，进行称为通气测试的测试。典型的鱼腥味被认为是阳性嗅觉测试，提示细菌性阴道病。
失去酸度。为了控制细菌的生长，阴道通常呈弱酸性，pH值为3.8–4.2。将排出的棉签放在石蕊试纸上以检查其酸度。 pH值大于4.5被认为是碱性的，提示细菌性阴道病。
在湿式安装中存在线索细胞。与嗅觉测试相似，对线索细胞的测试是通过将一滴氯化钠溶液放在载有白带的载玻片上进行的。如果存在，可以在显微镜下观察线索细胞。之所以这么称呼它们，是因为它们提供了放电原因的线索。这些是被细菌覆盖的上皮细胞。
除放电本身外，两个阳性结果也足以诊断BV。如果没有排出，则需要满足所有三个条件。[需要非主要来源]细菌性阴道病的鉴别诊断包括以下内容：

正常白带。
念珠菌病（鹅口疮或酵母菌感染）。
滴虫病，一种由阴道滴虫引起的感染。
有氧性阴道炎
疾病控制中心（CDC）将性传播感染定义为“由可通过性活动获取和传播的病原体引起的各种临床综合征和感染”。但是CDC并未明确将BV识别为性传播感染。

Amsel标准
在临床实践中，可以使用Amsel标准诊断BV：

稀薄，白色，黄色，均匀放电
显微镜下的线索细胞
阴道液的pH值> 4.5
加入碱（10％氢氧化钾（KOH）溶液）时，释放出腥味。
四个标准中至少应有三个用于确诊。 Amsel标准的修改接受两个因素而不是三个因素的存在，并且被认为具有同等诊断意义。

革兰氏染色
一种替代方法是使用符合Hay / Ison标准或Nugent标准的革兰氏染色阴道涂片。 Hay / Ison标准定义如下：

1级（正常）：乳杆菌的形态类型占主导地位。
2级（中级）：存在一些乳杆菌，但也存在Gardnerella或Mobiluncus形态型。
3级（细菌性阴道病）：主要是加德纳菌和/或Mobiluncus形态型。乳酸菌很少或没有。（Hay et al。，1994）
阴道加德纳菌是BV的主要元凶。阴道加德纳菌是短杆（coccobacillus）。因此，线索细胞和革兰氏可变球菌的存在指示或诊断细菌性阴道病。

分数
由于读取幻灯片需要花费时间，因此医生很少使用Nugent评分，并且需要使用训练有素的显微镜专家。通过组合其他三个分数可以生成0-10的分数。分数如下：

0–3被视为BV的负数
4–6被视为中级
7+被认为是BV的指标。
至少要计数10–20个高功率（1000次油浸）场并确定平均值。

使用Nugent标准将Affirm VPIII的DNA杂交测试与革兰氏染色进行比较。 Affirm VPIII测试可用于有症状女性的BV快速诊断，但使用昂贵的专有设备来读取结果，并且无法检测到导致BV的其他病原体，包括Prevotella spp，Bacteroides spp和Mobiluncus spp。

筛选
从2020年开始，美国不建议在怀孕期间进行筛查。

预防
建议降低风险的一些步骤包括：不要强迫，避免性行为或限制性伴侣的数量。

一项评论得出结论，益生菌可能有助于防止再次发生。另一则评论发现，尽管有初步证据，但仍不足以推荐将其用于此目的。

早期证据表明，对男性伴侣进行抗生素治疗可以重新建立男性泌尿生殖道的正常菌群，并防止感染的复发。但是，2016年的Cochrane评估发现高质量证据表明，治疗患有细菌性阴道病的女性的性伴侣对受影响的女性的症状，临床结局或复发没有影响。还发现这种治疗可能导致治疗的性伴侣报告不良事件增加。

治疗
抗生素类
通常用抗生素甲硝唑或克林霉素治疗。它们可以通过口给药或在阴道内使用。但是，大约有10％到15％的人在使用抗生素的第一疗程后并没有改善，据报道复发率高达80％。在相同的治疗前/治疗后伴侣中进行性行为，复发率会增加，并且使用避孕套的时间不一致，尽管含雌激素的避孕药会降低复发率。如果在妊娠22周之前对有BV症状的孕妇给予克林霉素，则在妊娠37周之前早产的风险会降低。

其他可能起作用的抗生素包括大环内酯类，林可酰胺，硝基咪唑和青霉素。

细菌性阴道病不被视为性传播感染，不建议治疗细菌性阴道病女性的男性性伴侣。

益生菌
2009年的Cochrane审查发现，暂时性的但不足以证明益生菌可作为BV的治疗方法。 2014年的审查得出了相同的结论。 2013年的一项审查发现一些证据支持在怀孕期间使用益生菌。 BV的首选益生菌是阴道内含有高剂量乳酸菌（约109 CFU）的益生菌。阴道内给药优于口服。长期重复治疗似乎比短期治疗更有希望。

过氧化氢
一项研究发现，使用过氧化氢（3％）进行阴道冲洗可略有改善，但这远不及使用甲硝唑。
