体征和症状

血管性动脉疾病的体征和症状是基于受影响的身体部位。受PAD影响的患者中约有66％没有症状或有非典型症状。最常见的症状是间歇性c行，在行走或锻炼时会引起疼痛和严重的痉挛。疼痛通常位于患病腿的小腿肌肉中，休息后即可缓解。发生这种情况的原因是，在锻炼过程中，腿部肌肉需要更多的氧气，而在未受影响的腿部中，动脉将能够增加血液量，从而增加流向锻炼的腿部的氧气。但是，当变窄时，动脉无法满足肌肉对氧气增加的需求。

对于严重的PAD患者，可能会出现并发症，包括严重肢体缺血和坏疽。当动脉中血流的阻塞达到一定程度时，血液无法维持静止状态下组织的氧合，就会发生严重肢体缺血。这会导致休息时疼痛，感觉寒冷，或受影响的脚和脚趾麻木。严重PAD的其他并发症包括下肢组织丢失、动脉功能不全溃疡、勃起功能障碍和坏疽。糖尿病患者脚部坏疽的发病率是正常人的30倍。许多严重的并发症是不可逆的。

风险因素

导致PAD风险增加的因素与动脉粥样硬化的因素相同。这些因素包括年龄、性别和种族。PAD在男性的发病率是女性的两倍。就种族而言，PAD在有色人种中比白人更常见，比例为2:1。

吸烟,烟草使用在任何形式的最大风险因素是外周动脉疾病。根据剂量反应关系，吸烟者患外周PAD的风险增加了10倍。暴露在二手烟环境中也被证明会促进血管内壁(内皮)的变化，从而导致动脉粥样硬化。吸烟者患下肢PAD的几率是冠心病的2-3倍。大于80%-90%的下肢外周动脉疾病患者是目前或以前的吸烟者。患外周PAD的风险随着每天吸烟的次数和吸烟年限的增加而增加。

高血糖-糖尿病被证明会增加2-4倍的PAD风险。它通过引起外周动脉内皮细胞和平滑肌细胞功能障碍来实现这一点。发展下肢外周动脉疾病的风险与糖尿病的严重程度和持续时间成正比。

高胆固醇-血脂异常，是血液中胆固醇或脂肪的异常高水平。血脂异常是由高水平的低密度脂蛋白(LDL胆固醇)、低水平的高密度脂蛋白(HDL胆固醇)、总胆固醇升高和/或高甘油三酯水平引起的。这血胆固醇水平异常与加速外周动脉疾病。通过饮食、运动和/或药物治疗来控制血脂异常，可以显著降低心脏病发作和中风的发生率。

高血压——高血压或血压升高会增加患外周动脉疾病的风险。与PAD类似，高血压与心脏病、中风和腹主动脉瘤之间也有已知的联系。高血压可使男性和女性发生间歇性跛行的风险分别增加2.5- 4倍。间歇性跛行是外周PAD最常见的症状。

机制

如前所述,最常见的原因外周动脉疾病,尤其是在病人超过40岁,是动脉粥样硬化。动脉粥样硬化是由脂质或脂肪堆积和受影响动脉壁中的钙沉积引起的动脉狭窄。最常受影响的部位发生在动脉分支点，因为在这些动脉分支供应远端结构的地方，湍流和压力增加了。远端结构的疾病，包括脚和脚趾，通常由糖尿病引起，见于老年人。

额外机制外周动脉疾病包括动脉痉挛、血栓形成和纤维肌性的发育不良。动脉痉挛的机制仍在研究中，但它可以继发于创伤。当动脉痉挛或压迫自身造成阻塞时，就会出现跛行症状。与动脉粥样硬化相似，这导致流向阻塞下游组织的血液减少。血栓形成，或血栓的形成，通常是由于淤血或创伤。血管内壁的损伤开始了血栓形成的过程。血凝块最终导致动脉变窄，阻止足够的血液和氧气流向组织。

诊断

诊断或鉴定外周动脉疾病需要历史的症状和体检之后,确认测试。设置的症状符合外周动脉疾病医生将检查个别特定的考试结果。异常的体检结果可能导致医疗保健提供者考虑一个特定的诊断。然而，为了确认诊断，需要进行确认性测试。

当受累肢体处于抬高位置时，Buerger测试可以检查是否出现苍白。然后将肢体从抬高位置移至坐位，检查是否发红，即所谓的反应性充血。Buerger的测试是对动脉充分性的评估，也就是动脉向组织供应含氧血液的能力。

怀疑如果外周动脉疾病,最初的研究是踝肱指数(ABI)。ABI是一种简单、无创的测试，它测量踝关节收缩压与上臂收缩压的比值。这是基于这样一种观点，即如果踝关节的血压读数低于手臂，则怀疑从心脏向踝关节供血的动脉存在堵塞。

ABI范围在0.90到1.40被认为是正常的。当ABI≤0.90时，认为一个人有PAD。但是，如果ABI在0.41 - 0.90之间，PAD可进一步分级为轻度至中度，如果ABI小于0.40，则可分级为重度。这些分类可以帮助我们了解病程。此外，ABI值0.91到0.99被认为是边界，>1.40表示不可压缩动脉。如果按ABI >1.40计算，这可能表明钙化引起的血管壁僵硬，这可能发生在糖尿病患者中。ABIs(>1.40)异常高通常被认为是假阴性，因此，这样的结果值得进一步研究和更高水平的研究。动脉不可压缩的个体在两年内心血管疾病死亡的风险增加。

疑似PAD患者，ABIs正常，可进行ABI运动测试。基线ABI是在运动前获得的。然后要求患者进行运动(通常要求患者在跑步机上以恒定的速度行走)，直到发生跛行疼痛(最多持续5分钟)，之后再次测量脚踝压力。ABI降低15%-20%可诊断为PAD。

如果ABIs异常，下一步通常是下肢多普勒超声检查梗阻部位和动脉粥样硬化程度。其他的成像可以通过血管造影术进行，将导管插入股总动脉并选择性地引导到相关动脉。注射放射致密造影剂时，要拍x光片。在x光检查中发现的任何限制血流的堵塞都可以被发现并通过包括动脉粥样硬化切除术、血管成形术或支架植入术进行治疗。造影血管造影术是最容易获得和广泛使用的成像技术。现代计算机断层摄影(CT)扫描仪提供直接成像的动脉系统，作为替代血管造影术。

磁共振血管造影术(MRA)是一种非侵入性的诊断程序，它结合使用一个大型磁铁、无线电频率和计算机来产生体内血管的详细图像。MRA的优点包括其安全性和提供全腹部、骨盆和下肢高分辨率三维成像的能力。

分类

两种最常用的分类方法外周动脉疾病是方丹和卢瑟福的分类系统。1954年Rene Fontaine提出了方丹阶段，用来定义慢性肢体缺血的严重程度:

I期:无症状

IIa期:行走200余米后出现间歇性跛行

IIb期:步行不足200米后出现间歇性跛行

第三阶段:休息痛

第四阶段:肢体溃疡或坏疽

卢瑟福分类由美国血管外科学会和国际心血管外科学会提出，于1986年引入，1997年修订(以第一作者罗伯特·b·卢瑟福命名，称为卢瑟福分类)。该分类体系分为4个等级和7个类别(0-6类):

0级，0类:无症状
一级，第1类:轻度跛行
一级，第2类:中度跛行
三级:严重跛行
II级，第4类:休息痛
III级，第5类:轻微组织损失;不超过足指溃疡的局部缺血性溃疡
IV级，第6类:主要组织损失;严重的缺血性溃疡或坏疽

根据方丹(Fontaine)的III至IV级或卢瑟福(Rutherford)的4至5级，中度至重度外周动脉垫层以严重肢体缺血的形式出现肢体威胁(肢体丧失的风险)。

近日，美国血管外科学会(Society for Vascular Surgery)提出了“伤口、缺血、足部感染(WIfI)”的分类体系。2013年发布的这一分类系统是为了说明过去40年人口结构的变化，包括高血糖发病率的增加，以及血管重建技术和能力的发展。这个系统是建立在缺血和血管造影疾病模式的基础上，而不是截肢风险的唯一决定因素。WIfI分类系统分为两部分:伤口和缺血。根据有无溃疡和/或坏疽和局部缺血，将伤口分级为0到3级。

0级:无溃疡，无坏疽
1级:小、浅性溃疡;没有坏疽
2级:深部溃疡，肌腱或骨暴露，坏疽局限于脚趾
3级:广泛性全层溃疡;坏疽延伸到前足或中足

根据ABI、踝关节收缩压和脚趾压力，将缺血分为0到3级。

0级:踝部ABI 0.8或更高
1级:肱动脉指数0.6 ~ 0.79，踝关节压力70 ~ 100mm Hg，脚趾压力40 ~ 59mm Hg
2级:ABI 0.4-0.59，脚踝压力50 - 70毫米汞柱，脚趾压力30 - 39毫米汞柱

TASC(和TASC II)分级建议的PAD治疗是基于血管造影所见疾病的严重程度。

筛选

目前尚不清楚在一般人群中筛查疾病是否有用，因为它尚未得到适当的研究。这包括用踝肱指数进行筛查。

对冠状动脉疾病或颈动脉疾病进行检测是否有益尚不清楚。虽然PAD是腹主动脉瘤(AAA)的危险因素，但没有数据显示有无症状PAD的个体是否存在腹主动脉瘤。在有症状的PAD患者中，超声筛查AAA是不合理的。

治疗

根据疾病的严重程度，可根据以下准则采取以下步骤:

对于那些跛行患者，经常锻炼可以帮助打开可选的小血管(侧支流)，从而改善行走的局限性。跑步机运动(35到50分钟，每周3到4次)被认为是另一种有许多积极结果的治疗方法，包括减少心血管事件和提高生活质量。有监督的运动项目增加患者的无痛行走时间和最大行走距离。

药物治疗

糖尿病管理

高血压的管理

管理高胆固醇和抗血小板药物，如阿司匹林和氯吡格雷。他汀类药物分别可以减少血栓形成和胆固醇水平，有助于疾病进展，并解决患者可能会有的其他心血管风险。

根据指南,服用阿司匹林或氯吡格雷建议减少心肌梗死,中风和血管性死亡的其他原因与症状外周动脉疾病。阿司匹林和氯吡格雷在有症状的PAD患者中降低心血管缺血性事件风险的有效性尚不明确。

西洛他唑可以改善一些症状。五酮福林的益处尚不清楚。西洛地唑可以提高步行的人经验跛行由于外周动脉疾病,但没有强有力的证据表明,它提高了生活质量,降低死亡率,或减少心血管事件的风险。

使用其他药物或维生素治疗没有临床证据支持，“但评估叶酸和维生素B12对高同型半胱氨酸血症(一种公认的血管危险因素)影响的试验已接近完成。”

血管再生

在试验了上述最好的治疗方案后，如果症状仍然存在，患者可能会被推荐给血管或血管内外科医生。血运重建的益处被认为与缺血的严重程度以及伤口和感染的严重程度等其他导致肢体丧失的危险因素的存在相对应。

血管成形术(或经皮腔内血管成形术)可用于大动脉的孤立病变，如股动脉，但可能没有持久的效果。血管成形术后的通畅率最高的是髂动脉，而随着向脚趾的动脉而降低。影响血运重建结果的其他标准是病变长度和病变数量。在血管成形术后放置支架以保持狭窄的收窄动脉开放似乎没有长期的优势或持续的好处。

血管旁路移植术可以用来绕过动脉血管的病变区域。如果有足够的静脉导管，可以使用大隐静脉作为导管，虽然人工材料(Gore-Tex或PTFE)在没有足够的静脉导管的情况下经常用于长时间的移植。

当坏疽出现时，可能需要截肢，以防止感染组织引起败血症，一种危及生命的疾病。

2013年，美国心脏病学会和美国心脏协会联合2005年和2011年的指南编制了下肢、肾脏、肠系膜和腹主动脉垫的诊断和治疗指南。对于有威胁的慢性肢体缺血，ACCF/AHA指南建议球囊血管成形术仅适用于预期寿命在2岁或以下的人，或者没有自体静脉可用的人。对于预期寿命超过2年或有自体静脉的患者，建议进行搭桥手术。

预后

PAD患者“发生心血管事件的风险格外高，大多数人最终将死于心脑血管原因”;预后与ABI测量的PAD严重程度相关。大血管垫显著增加心血管疾病的死亡率。PAD在10年内发生冠状动脉事件的风险超过20%。

患有跛行的患者发生严重缺血并需要截肢的风险很低，但冠状动脉事件导致死亡的风险是没有跛行的对照组的三到四倍。在间歇性跛行患者中，只有“7%将进行下肢旁路手术，4%的截肢，16%的跛行恶化”，但中风和心脏病发作事件有所增加，“5年死亡率估计为30%(对照组为10%)”。

流行病学

外周动脉疾病在一般人群中的患病率为12-14%，70岁以上人群中患病率高达20%;70%-80%的感染者无症状;只有少数人需要进行血管重建或截肢。外周动脉疾病影响了三分之一的糖尿病患者50岁以上的。在美国，65岁及以上的美国人有12%至20%患此病。大约有1000万美国人有PAD。尽管其发病率和心血管危险的含义，只有25%的外周动脉疾病患者正在接受治疗。

2000年，在美国40岁及以上的人群中，外周动脉疾病的发病率为4.3%。70岁或70岁以上的人有14.5%的比率。在年龄组中，女性的发病率普遍高于男性。非西班牙裔黑人的比率为7.9%，而非西班牙裔白人为4.4%，墨西哥裔美国人为3.0%(1.4%-4.6%)。

症状性外周动脉疾病的发病率随着年龄的增长而增加，从40-55岁男性每年约0.3%到75岁以上男性每年约1%。外周动脉疾病的流行程度在很大程度上取决于外周动脉疾病的定义和被研究人群的年龄。诊断是至关重要的，因为PAD患者心脏病发作或中风的风险要高出4到5倍。

分别在1型和2型糖尿病患者中进行的糖尿病控制和并发症试验以及英国前瞻性糖尿病研究试验表明，血糖控制与微血管疾病的相关性大于大血管疾病。在小血管中发生的病理改变可能比在大血管中发生的动脉粥样硬化对长期升高的血糖水平更敏感。

研究

目前正在研究预防PAD进展的治疗方法。对于那些腿部血流量严重不足的患者，自体移植自体单核细胞的益处尚不清楚。

仅进行了一项比较血管旁路和血管成形术治疗严重垫层的随机对照试验。该试验发现，在计划的临床终点，血管旁路术和血管成形术在无截肢生存期上没有差异，但该试验被批评为动力不足、限制血管内选择和比较不合适的终点。截至2017年,两个随机的临床试验正在进行,以更好地了解最优危重肢体缺血严重垫和血管再生技术(CLI)的BEST-CLI(最佳血管内和最佳手术治疗患者危重肢体缺血)试验,和BASIL-2(旁路与腿部的血管成形术在严重缺血- 2)试验。
