肝脏表现出异质性、边缘不规则性，有多个实质结节，与肝硬化一致。

门静脉早期强化，提示肝内动门静脉分流。血栓占据门静脉主干及其左右分支，血管丰富提示癌栓。

肝右叶有无数结节的边界不清的肿块显示门静脉期不均匀的动脉强化和冲洗。左叶中的一些富血管动脉结节在门脉期也显示出消失。

在门静脉期，肿瘤可以被准确地识别为异质低密度（红星）与相邻的非癌组织（绿星）。

通过经血管途径继发于 HCC 的肝内动脉门静脉分流：在肝动脉期，来自肝动脉分支的血管滋养管穿过门静脉壁，在肿瘤血栓处形成线和条纹征（粉红色箭头）。

案例讨论
CT 表现符合弥漫性肝细胞癌 (HCC)，伴动门静脉分流和恶性门静脉血栓形成。

由于大量造影剂通过分流器流入门静脉，在严重的动门静脉分流情况下，HCC的强化会显著降低。在平衡阶段，仍然可以准确识别肿瘤的特征性冲洗。平衡相对于将 HCC 诊断和描述为异质低密度区域最有用。
